摘要
目的 探讨应用双套管冲洗在低位直肠癌保肛术后治疗吻合口漏的疗效及价值.方法 选取2014年03月—2017年12月诊治的直肠癌保肛术后发生吻合口漏患者52例,随机分为两组,对照组24例患者术后采取传统引流袋被动引流,观察组28例患者术后采用盆底放置双套管冲洗并负压引流.比较两组患者术后的恢复情况和治疗效果.结果 观察组患者体温恢复正常时间、血象恢复正常时间及拔管时间均小于对照组(P〈0.05),观察组总有效率(100%)高于对照组(83.3%).结论 应用双套管行冲洗并负压引流更有效促进低位直肠癌保肛术后吻合口漏的愈合,缩短患者术后恢复时间,显示出更好的临床疗效.
Objective To explore and evaluate the clinical application value of double-lumen cannula for the treatment of anastomotic leakage after anus-preserving operation for lower rectal cancer. Methods We retrospectively analyzed a total of 52 low rectal cancer patients with anastomotic leakage after anus-preserving operation in our hospital during March 2014 to December 2017. According to the difference of prevention to anastomotic leakage, these patients were divided into two groups randomly that 24 cases used conventional drainage regard as control group, and 28 cases used double-lumen cannula perfusion accompanied with negative pressure drainage as study group. Postoperative recovery and therapeutic effect were compared between these two groups. Results The times returned to normal of body temperature and hemogram indices, the time of postoperative extubation of study group were shorter and lower than those of control group respectively (P 〈 0.05). The total effective rate of study group (100%) was higher than control group (83.3%) as well. Conclusion The application of double-lumen cannula perfusion accompanied with negative pressure drainage can accelerate effectively the healing of anastomotic leakage after anus-preserving operation for lower rectal cancer and shorten postoperative recovery time, revealing better clinical treatment effect.
