摘要
目的:比较寒湿证和湿热证骨关节炎(OA)患者关节腔积液量,以及组织中基质金属蛋白酶13(MMP13)、一氧化氮(NO)、肿瘤坏死因子(TNF-α)及转化生长因子-β1(TGF-β1)表达水平的差异性。方法:选择本院2015年5月—2016年12月收治的100例湿热证OA患者(观察1组)和100例寒热证OA患者(观察2组),均行患肢膝关节核磁共振成像(MR)检查,比较关节腔积液量。再随机选择70例行膝关节置换术的OA患者,分为湿热组30例,寒湿组30例和正常对照组10例,比较其组织中MMP13、NO、TNF-α及TGF-β1表达水平。结果:(1)观察1组患者关节腔积液厚度显著大于观察2组(P〈0.05)。(2)湿热组MMP13阳性率高于寒湿组和正常对照组(P〈0.05),寒湿组MMP13阳性率显著高于正常对照组(P〈0.05);湿热组NO水平高于寒湿组和正常对照组(P〈0.05),寒湿组NO水平显著高于正常对照组(P〈0.05)。(3)湿热组TNF-α阳性率高于寒湿组和正常对照组(P〈0.05),TGF-β1阳性率显著低于寒湿组和正常对照组(P〈0.05);寒湿组TNF-α阳性率高于正常对照组(P〈0.05),TGF-β1阳性率显著低于正常对照组(P〈0.05)。结论:湿热证OA患者关节腔积液量较寒湿证OA患者高,MMP13、TNF-α阳性表达率和NO水平高于寒湿证OA患者,TGF-β1阳性表达低于寒湿证OA患者。
Objective:To compare the differences of the joint effusion volume and the expression levels of tissue matrix metalloproteinase 13( MMP13),nitric oxide( NO),tumor necrosis factorα( TNF-α) and transforming growth factor-β1( TGF-β1)incold-dampness syndrome and damp-heat syndrome of osteoarthritis( OA) patients. Methods:A hundred cases of damp-heat syndrome OA patients( the observation group1) and 100 cases of cold-dampness syndrome OA patients( the observation group2) treated in our hospital from May 2015 to December 2016 were selected and were treated with limb knee joint magnetic resonance imaging( MR) examination,and the joint effusion volume was compared. The other 70 cases of OA patients with knee arthroplasty were randomly selected and divided into the damp-heat group( n = 30),the cold-dampness group( n = 30) and the normal control group( n = 10). The expression levels of MMP13,NO,TNF-α and TGF-β1 in tissues were compared. Results:(1) The thickness of joint effusion volume in the observation group 1 was significantly higher than that in the observation group 2(P〈0.05).(2) The positive rate of MMP13 in the damp-heat group was higher than that in the cold-dampness group and in the normal control group( P〈0.05),and the rate in the cold-dampness group was significantly higher than that in the normal control group( P〈0.05). The level of NO in the damp-heat group was higher than that in the cold-dampness group and in the normal control group( P〈0.05),and the level in the cold-dampness group was significantly higher than that in the normal control group( P〈0.05).(3)The TNF-α positive rate in the damp-heat group was higher than that in the cold-dampness group and the normal control group( P〈0.05) while the TGF-β1 positive rate was significantly lower than that in the cold-dampness group and the normal control group( P〈0.05).
