摘要
名中医张杰教授学验俱丰,认为"风、寒(热)、湿"等外邪致病虽为类风湿关节炎的基本病机,外湿是其重要的致病因素之一,但是外湿易伤及脾阳,脾受湿困,引动内湿,形成内生痰湿,强调脾虚、内生痰湿是类风湿关节炎关键的致病因素。基于这两点致病因素,针对类风湿关节炎病程较长、兼痰湿体质者,在治法方面,张杰教授认为治法以祛痰除湿,与祛风、通络、散寒、止痛并重,兼顾补脾以扶助正气;在用药方面,以陈皮、半夏为核心药对,注重运用祛痰化湿兼顾补脾类方药,临床取得较好的疗效。文章阐释类风湿关节炎的病机特点,总结归纳了张杰教授数十载诊治"脾虚生湿"型类风湿关节炎患者的学术体系及思想、临证经验,具有较高临床参考价值,值得学者借鉴与推广。
Professor ZHANG Jie,a famous Chinese medicine professor,has high academic ability and rich clinical experience,she believes that although exogenous pathogenic factors,such as“wind,cold(heat)and dampness”,are the basic pathogenesis of rheumatoid arthritis,and external dampness is one of the important pathogenic factors of rheumatoid arthritis,external dampness injures the spleen,induces internal dampness,and forms endogenous phlegm dampness.It is emphasized that insufficiency of the spleen and phlegm dampness are the key pathogenic factors of the disease.Based on these two pathogenic factors,for the patients with long course of rheumatoid arthritis and phlegm dampness constitution,professor ZHANG Jie thinks that the treatment method is to remove phlegm and dampness,pay equal attention to expelling wind,dredging collaterals,dispersing cold and relieving pain,and give consideration to invigorating spleen to support healthy Qi;in terms of medication,taking Chenpi(Citri Reticulatae Pericarpium)and Banxia(Pinelliae Rhizoma)as the core drug pair,paying attention to the use of expectorant and dampness,giving consideration to invigorating spleen curative effect.This paper explains the pathogenesis characteristics of rheumatoid arthritis,summarizes professor ZHANG Jie’s academic system and ideas,clinical experience in the diagnosis and treatment of rheumatoid arthritis patients with“spleen deficiency and dampness”for decades,which has high clinical reference value and is worthy of reference and promotion.
