阿魏酸哌嗪对糖尿病肾病患者肾功能、内皮功能及氧化应激的影响 被引量:12

Effect of piperazine ferulate on renal function, endothelial function and oxidative stress in patients with diabetic nephropathy

摘要 目的探讨阿魏酸哌嗪对糖尿病肾病患者肾功能、内皮功能及氧化应激的影响。方法选取2017年1~6月期间在自贡市第一人民医院诊治的80例糖尿病肾病患者,以随机数表法分为对照组与观察组,每组40例,对照组给予低蛋白饮食、适当运动、戒烟戒酒、降糖药物口服等治疗,观察组在对照组治疗基础上加用阿魏酸哌嗪片口服,两组疗程均为3个月。比较两组患者治疗前后的血肌酐(Scr)、尿素氮、24 h尿蛋白、尿β2微球蛋白(β2-MG)、尿蛋白排泄率(UAER)、血清一氧化氮(NO)、内皮素-1(ET-1)、肿瘤坏死因子-α(TNF-α)、白细胞介素6和8(IL-6、IL-8)的水平,并对治疗后的疗效进行评价。结果 Scr、尿素氮、24 h尿蛋白、β2-MG、UAER两组组内比较治疗后较治疗前下降,差异均有统计学意义(P<0.05),治疗后观察组患者的Scr、尿素氮、24 h尿蛋白、β2-MG、UAER分别为(72.34±10.78)μmol/L、(5.27±2.36)mmol/L、(224.67±32.85)mg/24 h、(0.38±0.13)mg/L、(62.29±5.44)μg/min,对照组分别为(89.13±11.25)μmol/L、(6.71±2.94)mmol/L、(257.11±37.62)mg/24 h、(0.56±0.11)mg/L、(73.04±5.61)μg/min,观察组较对照组下降更明显,差异均有统计学意义(P<0.05);NO和ET-1两组组内比较NO治疗后较治疗前升高、ET-1治疗后较治疗前下降,差异均有统计学意义(P<0.05),治疗后观察组患者的NO和ET-1分别为(69.46±10.42)μmol/L、(72.36±6.49)pg/L,对照组分别为(57.87±8.12)μmol/L、(83.20±8.25)pg/L,治疗后观察组较对照组NO升高、ET-1下降更明显,差异均有统计学意义(P<0.05);TNF-α、IL-6、IL-8两组组内比较治疗后较治疗前下降,差异均具有统计学意义(P<0.05),治疗后观察组患者的TNF-α、IL-6、IL-8分别为(35.38±12.81)ng/L、(12.41±3.42)ng/L、(121.01±22.93)ng/L,对照组分别为(47.63±13.55)ng/L、(16.27±3.57)ng/L、(141.80±27.64)ng/L,治疗后观察组较对照组下降更明显,差异均有统计学意义(P<0.05);观察组治疗总有效率� Objective To investigate the effects of piperazine ferulate on renal function, endothelial function and oxidative stress in patients with diabetic nephropathy. Methods A total of 80 patients with diabetic nephropathy treated in Zigong First People's Hospital during January 2017 and June 2017 were investigated. They were divided into the control group and the observation group by a random number table method(40 cases in each group). The control group was given a low protein diet, moderate exercise, abstaining from tobacco and drink and oral administration of hypoglycemic drugs; the observation group was given oral administration of piperazine ferulate tablets on the basis of the treatment of the control group; the course of treatment was 3 months. The serum creatinine(Scr),urea nitrogen, 24 h urine protein, urinary beta 2 microglobulin(β2-MG), urinary albumin excretion rate(UAER), serum nitric oxide(NO), endothelin 1(ET-1), tumor necrosis factor alpha(TNF-α), interleukin 6 and 8(IL-6, IL-8)levels of the patients were compared between the two groups before and after treatment, and the therapeutic effects were evaluated after treatment. Results Scr, urea nitrogen, 24 h urine protein, β2-MG, UAER were compared between the two groups, which decreased significantly after treatment compared with before treatment(P0.05); and after treatment, the Scr, urea nitrogen, 24 h urine protein, β2-MG and UAER of the observation group were(72.34±10.78) μmol/L,(5.27±2.36) mmol/L,(224.67±32.85) mg/24 h,(0.38±0.13) mg/L,(62.29±5.44) μg/min, respectively,compared to(89.13±11.25) μmol/L,(6.71±2.94) mmol/L,(257.11±37.62) mg/24 h,(0.56±0.11) mg/L,(73.04±5.61) μg/min of the control group, respectively; the levels in observation group decreased more obviously than those in the control group,and the difference was statistically significant(P0.05).
