摘要 目的:了解泸州地区侵袭性肺炎链球菌耐药情况,为临床合理使用抗生素提供依据。方法:回顾性分析我地区2015年8月至2017年7月确诊的95例侵袭性肺炎链球菌感染病例的流行特征及耐药情况。结果:侵袭性肺炎链球菌感染多发于冬春季节,血液感染是主要感染类型,易感人群主要是60岁以上老人及2岁以下婴幼儿;95株侵袭性肺炎链球菌中,对红霉素及克林霉素的耐药率高,达80%以上,其次是复方新诺明、四环素类和氯霉素,发现耐青霉素菌株1株;未发现对头孢菌素类、喹诺酮类、利奈唑胺、亚胺培南、万古霉素以及奎奴普丁耐药的菌株。结论:本研究中侵袭性肺炎链球菌对红霉素、克林霉素和复方新诺明耐药率高,但对青霉素敏感性仍较好,可以作为首选抗生素;同时应加强对其耐药性监测,指导临床合理使用抗生素。 Objective: To investigate drug resistance of invasive Streptococcus pneumoniae in Luzhou, China,and to provide a basis for rational use of antibiotics in clinical practice. Methods: A retrospective analysis was per-formed for the epidemiological characteristics and drug resistance of 95 patients with invasive Streptococcus pneu-moniae infection who were diagnosed from August 2015 to July 2017. Results: Invasive Streptococcus pneumoniaeinfection mainly occurred in spring and winter, and blood infection was the main infection type. The elderly aged 〉60 years and infants aged 〈2 years were the susceptible population. The 95 strains of invasive Streptococcus pneu-moniae had high rates of drug resistance to erythromycin and clindamycin, up to 〉80%, followed by cotrimoxazole,tetracycline, and chloramphenicol. One strain was resistant to penicillin, and there were no strains which were resis-tant to cephalosporins, quinolones, linezolid, imipenem, vancomycin, and quinupristin. Conclusion: Invasive Strep-tococcus pneumoniae strains have a high rate of drug resistance to erythromycin, clindamycin, and cotrimoxazole,while they have a good sensitivity to penicillin, and thus penicillin can be used as the preferred antibiotic. Meanwhile, drug resistance should be closely monitored to guide the rational use of antibiotics in clinical practice.
