肺康复对早期脑卒中患者的康复影响研究 被引量:9

Effect of pulmonary rehabilitation in patients early after stroke

摘要 目的评价早期脑卒中患者系统开展肺功能康复训练后的临床疗效。方法选择2014年1月至2015年6月脑卒中后在本科住院的患者80例随机分为两组,每组40例,其中,40名接受系统肺康复训练患者为治疗组,40名未进行系统肺康复训练患者为对照组。治疗2周后,比较两组患者治疗前后肺功能、日常生活活动能力(ADL)、6min步行距离;30d后,观察患者在治疗期间因肺部感染应用抗菌素的比率、因病情加重转入监护室的几率;结果两组对象基线资料无明显差异;对照组因肺部感染应用抗菌素治疗率为55%,明显高于治疗组25%,P〈0.05;训练前治疗组的VCmax%、FVCmax%、Baahel指数、6min步行距离分别由治疗前的54.8±20.30、47.9±12.75、33.76±11.46、213.7±121.9,治疗后上升到70.4±10.98、45.4±12.66、454.24±128.8、77.43±16.98,明显高于对照组,均P〈0.05。结论早期脑卒中患者接受系统的肺功能康复训练有利于降低抗菌素的使用率,提高患者运动能力及运动储能,并能够改善患者的生活能力。 Objective To evaluate the clinical effect of pulmonary rehabilitation in patients early after stroke. Methods Included in this study were 80 stroke patients who stayed in our hospital between January 2014 and June 2015. The patients were randomly divided into two groups (n=40 each). Forty patients in the treatment group received systemic pulmonary rehabilitation training, while the other 40 in the control group did not. After the 2-week treatment, the two groups were compared for lung function, daily living activity (ADL) and 6-minute walk distance before and after treatment. After 30 days, the proportions of patients on antibiotics and those who were transferred to ICU due to severe disease in the two groups were calculated. Results There was no significant difference in baseline between the two groups. In the control group, the rate of antibiotic treatment due to pulmonary infection was 55%, which was significantly higher than that in the treatment group (25%, P〈0.05). VCmax% , FVCmax% , Barthel index and 6-rain walk distance were 54.8 ± 20.30, 47.9 ± 12.75, 33.76 ± 11.46 and 213.7 ± 121.9 before treatment, respectively. These values increased after treatment to 70.4 ± 10.98, 45.4 ± 12.66, 454.24 ± 128.8, and 77.43 ± 16.98, respectively, which were significantly higher compared with the values in the control group (all P〈0.05). Conclusion Systemic pulmonary rehabilitation training early after stroke is helpful to reduce the use of antibiotics, increase the exercise capacity and energy reserve, and improve the patient activity of daily life.
