摘要
目的研究在应用锁定钢板治疗肱骨近端骨折时内侧柱用螺钉支撑与不用螺钉支撑的不同效果。方法回顾性分析2016年7月—2017年12月该院肱骨近端骨折患者70例,将其中内侧柱用螺钉支撑的患者共35例视作观察组,将另外内侧柱不用螺钉支撑的患者共35例视作对照组,比较效果。结果观察组肩关节优良率为91.43%,对照组肩关节优良率62.86%(P<0.05);观察组治疗后骨折愈合时间为(3.11±0.52)个月,肱骨头高度丢失值为(1.42±1.03)mm,对照组结果分别为(3.69±0.89)个月、(6.28±3.71)mm,(t=3.162 2、7.428 4,P=0.008 1、0.006 0);观察组没有患者出现并发症,对照组并发症发生率为17.14%(χ~2=6.329 5,P=0.001 7)。结论在应用锁定钢板治疗肱骨近端骨折时内侧柱用螺钉支撑能够提升稳定性,实现复位的更有效维持,可在临床推广。
Objective This paper tries to study the different effects of medial column support with and without screw support in the treatment of proximal humeral fractures with locking plate. Methods A retrospective analysis of 70 patients with proximal humeral fracture from July 2016 to December 2017 in the hospital was conducted. 35 patients with medial column supported by screws were treated as observation group and 35 patients with other medial column without screw support as control group. The results were compared. Results The rate of good shoulder was 91.43% in the observation group, 62.86%in the control group（P〈0.05）; the healing time of the observation group was（3.11 ±0.52） months after treatment, and the height of the humeral head was（1.42±1.03） mm, the control group results were（3.69±0.89） months,（6.28±3.71） mm,（t=3.1622, 7.428 4, P=0.008 1, 0.006 0）; there was no complication in the observation group, and the incidence of complications in the control group was 17.14%（χ^2=6.329 5, P=0.001 7）. Conclusion In the application of locking plate in the treatment of proximal humerus fractures when the medial column with screws can improve the stability and achieve a more effective maintenance of reduction, can be promoted in the clinic.
