摘要 目的:对黑龙江省东部地区某医院的肠杆菌科细菌做统计分析,通过细菌的分布特点和耐药性的分析,指导临床医生合理的用药。方法:收集2012~2017年期间黑龙江省某医院住院患者和门诊患者的标本对肠杆菌科细菌进行分离培养,用仪器VITEK 2和Walk Away 40 plus进行细菌鉴定和药敏试验,K-B进行药敏试验复核法。结果:在2012~2017年间,我院共收集3920株肠杆菌科细菌,1754株(44.74%)肺炎克雷伯菌,位居首位,1228株(31.33%)大肠埃希菌,309株(7.88%)阴沟肠杆菌、115株(2.93%)奇异变形杆菌及76株(1.94%)产气肠杆菌。统计分析可知肠杆菌科细菌来源最多的标本为痰液(51.79%,2030株),其次为血液标本(17.01%,667株),尿液标本(15.36%,602株),三者为其主要来源。肠杆菌科细菌标本数量来源最多的为急诊(12.30%,482株),呼吸科(12.24%,480株)和神经科(11.02%,432株),是肠杆菌科细菌来源的主要科室。药敏分析结果显示头孢西丁、阿米卡星、哌拉西林敏感性较好,耐药率范围为2.44%~22.61%。结论:肠杆菌科细菌有较高的分离率,而且耐药性逐渐增高,为防止菌株耐药性的产生,我们建议抗菌药物应被严格管理与合理使用。 Objective: To analysis the distribution and drug resistance of Enterobacteriaceae in a hospital in Heilongjiang province so as to guide doctors to use drugs reasonably. Methods: The specimens of inpatient and outpatient from 2012 to 2017 were selected for isolated culture of Enterobacteriaceae. and then use VITEK 2 and Walk Away 40 plus were used to identify and recheck the drug sensitivity, recheck drug test with K - B methods. Results: From 2012 to 2017, 3920 strains of Enterobacteriaceae were collected, including 1754 (44.74%) Kleb- siella pneumoniae, 1228 (31.33% ) Escherichia coli, 309 (7.88% ) Enterobacter cloacae, 115 strains of Proteus mirabilis (2.93% ) and 76 strains of Enterobacteriaceae (1.94%). The statistical analysis showed that the most sources of Enterobacteriaceae were sputum (51.79%, 2030 strains), blood samples (17.01%, 667 strains), and urine specimens (15.36%, 602 strains) , they were the main sources. The largest number of Enterobacteriaceae specimens is emergency (12.30%, 482 strains), Department of respiration (12.24%, 480 strains) and Neurology (11.02% , 432 strains) are also the main section of Enterobacteriaceae bacteria. Drug sensitivity analysis indicated that Amikacin, cefoxitin, and piperacillin sensitivity are effective, and the range of resistance rate is 2.44% 22.61%. Conclusion : The bacteria of Enterobacteriaceae have a high separation rate, and the drug resistance is higher than before. In order to prevent the emergence of strain resistance, we suggest that the antibiotics should be strictly managed and used.
