集束化护理在重症颅脑损伤行机械通气患者中的应用效果 被引量:73

Application effects of cluster nursing intervention in severe brain injury patients with mechanical ventilation

摘要 目的探讨实施集束化护理干预对重症颅脑损伤行机械通气患者神经功能及感染的影响,探讨最佳护理措施。方法选择北京同仁医院ICU?2014年7月—2016年7月收治的重症颅脑损伤行机械通气患者112例,随机分为观察组和对照组,每组56例。观察组予以集束化护理,对照组给予常规护理。比较两组患者的治疗情况、感染情况,应用临床神经功能缺损评分标准对两组神经功能恢复情况进行评定。结果观察组患者机械通气时间(3.10±0.56)d,入住ICU时间(6.00±2.35)d,住院费用(2.78±0.25)万元,均低于对照组,差异有统计学意义(t值分别为4.282、4.113、3.267;P〈0.01)。观察组患者的肺部感染率5.36%,对照组为21.43%,差异有统计学意义(χ2=6.235,P〈0.05)。干预后观察组患者神经功能评分(11.35±2.00)分,对照组(22.45±2.78)分,差异有统计学意义(t=9.264,P〈0.01)。结论实施集束化护理干预措施可降低重症颅脑损伤行机械通气患者的感染情况,促进神经功能恢复。 ObjectiveTo explore the application effects of cluster nursing intervention on neurological function and infection in severe brain injury patients with mechanical ventilation, and to explore better nursing strategies.MethodsA total of 112 severe brain injury patients with mechanical ventilation who were treated in ICU Department of Beijing Tongren Hospital from July 2014 to July 2016 were selected in this study. The participants were randomly divided into two groups. The control group (n=56) received routine nursing care while the intervention group (n=56) received cluster nursing intervention. The treatment condition and infection rate were compared between two groups using clinical Neurological Function Deficit Scale (NDS) .ResultsThe intervention group had significantly shorter mechanical ventilation time [ (3.10±0.56) d], shorter length of ICU stays [ (6.00±2.35) d], and less hospitalization expenses [ (2.78±0.25) ten-thousands yuan]compared with the control group (t=4.282, 4.113, 3.267; P〈0.01) . The infection rates were significantly lower in the intervention group compared with the control group (5.36% vs. 21.43%; χ2=6.235, P〈0.05) . Furthermore, the intervention group had significantly lower NDS scores compared with the control group [ (11.35±2.00) vs. (22.45±2.78) ; t=9.264, P〈0.01].ConclusionsCluster nursing intervention measures can significantly reduce the infection rate and improve neurological function recovery in severe brain injury patients with mechanical ventilation.
