摘要
目的探讨弹性髓内针和闭合复位外固定支架对GartlandⅢ型儿童肱骨髁上骨折的疗效差异。方法选取2014年10月至2016年12月本院收治的Gartland Ⅲ型肱骨髁上骨折患儿120例为研究对象,采用随机数表法将其分为试验组和对照组,每组各60例。对照组患儿接受闭合复位外固定支架治疗,试验组患儿接受弹性髓内针内固定治疗,随访4个月,比较两组患儿的疗效及并发症发生情况。结果试验组患儿手术时间及住院天数均显著短于对照组(P_均<0.05),治疗优良率显著高于对照组(P<0.05)。随访结果显示,两组患儿并发症主要包括肘内翻、复位丢失、神经损伤及切口感染,试验组患儿并发症总发生率显著低于对照组(P<0.05)。结论弹性髓内针内固定具有切口和创伤小、简便易行、固定性良好、并发症少、骨折愈合快的优势,其对儿童长骨骨折的治疗效果极佳,适合临床使用。
Objective To investigate elastic intramedullary nail and closed reduction external fixator in the treatment of Gartland type Ⅲ supracondylar fracture of humerus in children. Method A total of 120 children with Gartland Ⅲ supracondylar fracture of humerus admitted to our hospital from October 2014 to December 2016 were enrolled in the study, they were randomly divided into experimental group and control group, with 60 cases in each group. Children in control group received closed reduction external fixator, children in experimental group received elastic intramedullary nail internal fixation. They were followed up for 4 months. The efficacy and complications of the two groups were compared. Result The operation time and hospitalization time of experimental group were significantly shorter than those of control group（P_（all） 0.05）. The excellent and good rates of experimental group were higher than that of control group（P〈0.05）. The follow-up results showed that the complications of the two groups mainly included cubitus varus, loss of reduction, nerve injury and incision infection. The complication rates of experimental group were lower than that of control group（P〈0.05）. Conclusion Elastic intramedullary nail internal fixation has the advantages of small incision, small trauma, simple and easy to do, good fixation, less complications, and rapid fracture healing. It has excellent therapeutic effect on long bone fracture in children.
