摘要
目的探讨医院超说明书用药管理及持续改进模式,对超说明书用药进行规范管理促进临床合理用药。方法本院对超说明书用药进行分级管理。对分级管理中不良反应较大的药物,回顾性调查不良反应情况;对临床药学处方点评、门急诊药房与静脉用药调配中心审方过程中新发现的超说明书用药品种进行总结。结果初步建立超说明书用药管理模式和流程及持续改进模式,为超说明书用药管理提供参考。结论从保障患者用药安全,提高医疗机构合理用药水平上看,本院超说明书用药的管理与持续改进模式是种有益的尝试,政府相关部门应尽早出台相应的管理规范,以更好地为临床安全合理用药服务。
Objective To manage and improve the off-label drug use in our hospital, standardized the off-label drug use, and promote rational drug use. Methods The off-label drug use in our hospital was graded. The drugs with serious adverse reactions were investigated retrospectively. We also summarized the clinical pharmacy prescription analysis, the outpatient pharmacy and pharmacy intravenous admixture services reviewing prescription process, and the newly discovered varieties of off-label drug use. Results We initially established the management mode and the continuous improvement model of off-label drug use. Conclusion The management and continuous improvement of off-label drug use help ensure the safety of patient medication and improve the rational drug use. The government should set up corresponding regulations.
