摘要
目的了解陕西省89例耐药结核分枝杆菌多位点数目可变串联重复序列分析(VNTR)的基因多态性及近期传播情况。方法选取陕西省10个地市结核分枝杆菌耐药菌株89株,提取基因组DNA,采用聚合酶反应(PCR)对15个VNTR位点进行检测分析,基因分型聚类分析采用Bio-Numerics5.0软件。结果经基因聚类分析.可分4个基因群(I群、Ⅱ群、Ⅲ群和Ⅳ群),为4种基因群分别占7.87%(7/89)、3.37%(3/89)、87.64%(78/89)、1.12%(1/89)。成簇率为4.49%(4/89);近期感染率最小估计为2.25%(2/89)。15个VNTR位点中有4个位点的多态性较好(h>0.60),5个位点的多态性尚可(0.3≤h≤0.6),6个位点的多态性较差(h<0.3)。结论初步证实陕西省耐药结核分枝杆菌菌株存在明显的基因多态性,其主要流行型为Ⅲ群。
Objective To study the genotype distribution and transmission patterns of 89 drug resistant Mycobacterium tuberculosis（ MTB） clinical isolates from Shaanxi province with the variable number tandem repeats（ VNTR）. Methods 89 drug resistant Mycobacterium tuberculosis（ MTB） clinical isolates selected from Shaanxi province were cultured in Lowenstein-lensen medium,and then their genome DNAs were extracted. Polymerase chain reaction（ PCR） was used to detect the VNTR locus in the DNA samples,and the clustering analysis of genotypes was done with Bio-Numerics5. 0. Results The clustering analysis of genotype showed that 89 strains were categorized into 4 gene clusters（ genogroup I,II,Il I and IV）,in which 7. 87%（ 7/89） was belong to genogroup I,3. 37%（ 3/89） was genogroup II,87. 64%（ 78/89） was genogroup III,and 1. 12%（ 1/89） was genogroup IV. The cluster rate was 4. 49%（ 4/89） and the least estimated proportion of recent transmission was 2. 25%（ 2/89）. The polymorphism of 4 in 15 VNTR loci was better（ h 〉0. 60）,and 5 loci was acceptable（ 0. 3≤h≤0. 6）. The remaining 6 loci were less polymorphic（ h 〈0. 3）. Conclusion The drug resistant Mycobacterium tuberculosis shows obvious gene polymorphisms in Shaanxi province,and cluster type III is the predominant prevalent strain.
