压力可调式CPAP装置在单肺通气中的应用 被引量:4

摘要 目的 :探讨压力可调式CPAP装置对单肺通气患者动脉血氧分压(PaO_2)的影响。方法:选择某院2015年11月至2016年11月全麻开胸手术单肺通气患者40例,随机分为实验组(20例)和对照组(20例)。实验组在单肺通气时接压力可调式CPAP装置,术中视情况调节持续正压通气(continuous positive airway pressure,CPAP)压力、呼吸参数、呼气末正压通气(positive end expiratory pressure,PEEP)压力。对照组常规单肺通气,术中视情况调节呼吸参数、PEEP压力。2组患者分别于侧卧位双肺通气10 min、单肺通气30 min及手术结束复苏前抽取动脉血进行血气分析。采用SPSS 12.0统计软件进行统计学分析。结果:2组患者单肺通气前后PaO_2比较存在显著性差异(P<0.05)。实验组单肺通气30 min时PaO_2明显高于对照组,存在显著性差异(P<0.05);2组患者在双肺通气10 min时的PaO_2和手术结束复苏前的组内、组间比较差异无统计学意义(P>0.05)。结论:使用压力可调式CPAP装置给手术侧肺持续正压通气,能很好地提高单肺通气时的PaO_2,值得推广应用。 Objective To observe the effect of pressure-adjustable CPAP device on arterial partial pressure of oxygen(PaO2)in patients undergoing one lung ventilation. Methods Totally 40 cases of thoracic surgery under general anesthesia from November 2015 to November 2016 in some hospital were randomly divided into an experimental group and a control group.The patients in the experimental group went through one-lung ventilation with the pressure-adjustable CPAP device, and such indexes were regulated according to intraoperative conditions as CPAP, respiratory parameters and positive end expiratory pressure(PEEP). In the control group the patients underwent conventional one-lung ventilation when the respiratory parameters and PEEP were adjusted according to intraoperative conditions. The patients in both the groups had the arterial blood gas analyzed 10 min after two-lung ventilation, 30 min after one-lung ventilation and before postoperative recovery when they were at lateral position. SPSS 12.0 software was used to execute statistical analysis. Results The two groups had significant difference in PaO2 after one-lung ventilation(P〈0.05), the experimental group had PaO2 statistically different from that in the control group 30 min after one-lung ventilation(P〈0.05), and there were no obvious inter-and intragroup differences in the two groups 10 min after two-lung ventilation and before postoperative recovery(P〉0.05). Conclusion The pressure-adjustable CPAP device can improve the arterial partial pressure of oxygen in patients with one lung ventilation, and thus is worthy promoting practically.
