血Cys-C与尿β_2-MG、kim-1、M-ALB早期诊断糖尿病肾病的价值 被引量:8

The value of serum Cys-C and urine β_2-MG, KIM-1 and M-ALB in the early diagnosis of diabetic nephropathy

摘要 目的探究血清Cys-C与尿β_2-MG、kim-1、M-ALB联合检测在早期糖尿病肾病(DN)诊断中的应用。方法收集本院2015年11月~217年3月124例2型糖尿病患者,根据患者24 h尿微量白蛋白排泄率检查结果不同分NDN组(n=62)和DN组(n=62),另于同期选择58例健康体检者为对照组,均进行血清Cys-C、尿β_2-MG、尿kim-1、尿M-ALB等项目检测。结果 (1)DN组的血清Cys-C、尿β_2-MG、kim-1、M-ALB水平均显著优于NDN组、对照组(P<0.05);(2)血清Cys-C、尿β_2-MG、kim-1、M-ALB联合检测的灵敏度为90.32%、特异性95.16%、准确率92.74%、阳性预测值94.92%、阴性预测值90.77%,均明显高于任一项目单项检测的结果(P<0.05)。结论与单一项目检测相比,血清Cys-C与尿β_2-MG、kim-1、M-ALB联合检测的诊断价值更高,能够显著提高诊断准确率,成为早期DN诊断的主要方法。 Objective To explore the application of combined detection of serum Cys-C and urineβ_2-MG, KIM-1 and M-ALB in the diagnosis of early diabetic nephropathy(DN). Methods From November 2015 to March 217, 124 patients with type 2 diabetes were selected in our hospital. According to the results of 24 h urine microalbumin excretion rate were divided into NDN group(n=62) and group DN(n=62). At the same time, 58 healthy persons were selected as the control group. And all of them were detected for serum Cys-C, urine β_2-MG, urine KIM-1 and urine M-ALB.Results(1)The levels of serum Cys-C, urine β_2-MG, KIM-1 and M-ALB in group DN were significantly better than NDN group and control group(P0.05).(2)The sensitivity of the combined detection was 90.32%,the specificity was 95.16%, the accuracy was 92.74%, the positive predictive value was 94.92%, and the negative predictive value was 90.77%,which were significantly higher than the results of single item test of any item(P 0.05). Conclusion Compared with single detection, the combined detection of serum Cys-C and urinary β_2-MG, kim-1, M-ALB has higher diagnostic value, can significantly improve the diagnostic accuracy, and become the main method of early DN diagnosis.
