摘要
目的探讨胸腔镜微创术中采用瑞芬太尼联合异丙酚麻醉的血流动力学、麻醉质量。方法从2015年5月至2017年12月诊治的胸腔镜微创术患者中选取63例,并按照麻醉方案不同分组:对照组(n=31)应用芬太尼联合异丙酚麻醉,研究组(n=32)应用瑞芬太尼联合异丙酚麻醉。比较两组血流动力学指标、麻醉效果。结果研究组手术后1 h、拔管后5 min的血流动力学指标优于对照组(P<0.05);研究组Ramsay镇静评分高于对照组,拔管时间、睁眼时间均短于对照组(P<0.05);两组患者的VAS疼痛评分与不良反应发生率比较,差异不具有统计学意义(P>0.05)。结论胸腔镜微创术中采用瑞芬太尼联合异丙酚麻醉,患者血流动力学平稳,麻醉质量高、安全性高,值得临床推广应用。
Objective To investigate the hemodynamics and anesthesia quality of remifentanil combined with propofol in thoracoscopic minimally invasive surgery. Methods Form May 2015 to December 2017, 63 patients undergoing thoracoscopic minimally invasive surgery were selected and grouped according to different anesthesia programs, the control group(n=31)was given fentanyl combined with propofol anesthesia, the study group(n=32) was given remifentanil combined with propofol anesthesia. The hemodyanarnics parameters and anesthesia effects were compared between the two groups. Results The hemodynamic parameters in the study group were better than those in the control group of 1 h after surgery and 5 min after extubation(P〈0.05). The Ramsay sedation score in the study group was higher than that in the control group, the extubation time and waking time in the study group were shorter than those in the control group(P〈0.05). The differences in VAS pain scores and adverse reactions between the two groups were not statistically significant(P〉0.05). Conclusion The hemodynamics is stable, the anesthetic quality is high and safety is high in the use of remifentanil combined with propofol in thoracoscopic minimally invasive surgery,which is worthy of clincal application and promotion.
