初发2型糖尿病患者非酯化脂肪酸与胰岛素抵抗的关系 被引量:3

Relationship between Free Fatty Acid and Insulin Resistance in Newly Diagnosed Type 2 Diabetic Patients

摘要 目的研究初发2型糖尿病(T2DM)患者血清非酯化脂肪酸(FFA)与胰岛素抵抗的关系。方法选取2016年10月至2017年3月在首都医科大学附属北京朝阳医院内分泌科住院治疗的初发T2DM患者106例作为T2DM组,同时同期健康体检者60例作为正常对照组。收集两组受试者一般资料[年龄、身高、体质量、腹围、臀围,计算体质指数(BMI)];分别测定总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、三酰甘油(TG)、高敏C反应蛋白(hs-CRP)、糖化血红蛋白(Hb A1c)、非酯化脂肪酸(FFA)、空腹血糖、空腹胰岛素水平,并计算稳态模型评估的胰岛素抵抗指数(HOMA-IR)。分析FFA与胰岛素抵抗的关系。结果 T2DM组BMI、腹围、臀围、hs-CRP、TC、LDL-C、TG、FFA、HOMA-IR、Hb A1c均高于正常对照组[(25.7±2.2)kg/m2比(23.1±1.3)kg/m2,(93±7)cm比(82±6)cm,(99±8)cm比(90±5)cm,(3.4±0.5)mg/L比(1.1±0.4)mg/L,(5.0±1.4)mmol/L比(4.8±1.3)mmol/L,(3.1±1.0)mmol/L比(1.2±0.6)mmol/L,(2.7±1.8)mmol/L比(1.1±0.3)mmol/L,(0.65±0.26)mmol/L比(0.43±0.12)mmol/L,(4.7±2.4)比(1.2±0.4),(9.2±1.1)%比(4.2±0.5)%](P<0.05或P<0.01)。相关性分析结果显示,血清FFA与BMI、腹围、TC、TG、HOMA-IR、Hb A1c呈正相关(r=0.330,0.469,0.215,0.227,0.248,0.588,均P<0.05)。结论初发T2DM患者血清FFA水平与胰岛素抵抗呈正相关。 Objective To explore the relationship between serum free fatty acids( FFA) and insulin resistance in patients with newly diagnosed type 2 diabetes mellitus( T2 DM). Methods From Oct. 2016 to Mar. 2017,106 newly diagnosed T2 DM patients were included as a T2 DM group,60 healthy subjects as a normal control( NC) group from Department of Endocrinology,Beijing Chaoyang Hospital Affiliated to Capital Medical University. The general data( age,height,weight,abdominal circumference,hip circumference,BMI) were collected; the levels of total cholesterol( TC),low-density lipoprotein cholesterol( LDL-C),triglyceride( TG),high-sensitive C-reactive protein( hs-CRP),glycated hemoglobin( Hb A1 c),FFA,fasting blood glucose,and fasting insulin levels were detected. The homeostasis model assessment-insulin resistance index( HOMA-IR) was calculated. The relationship between FFA and insulin resistance was analyzed. Results The BMI,abdominal circumference,hip circumference,hs-CRP,TC,LDL-C,TG,FFA,HOMA-IR,Hb A1 cin the T2 DM group were all higher than those in the NC group [( 25. 7 ± 2. 2) kg/m2 vs( 23. 1 ± 1. 3) kg/m2,( 93 ± 7) cm vs( 82 ± 6) cm,( 99 ±8) cm vs( 90 ± 5) cm,( 3. 4 ± 0. 5) mg/L vs( 1. 1 ± 0. 4) mg/L,( 5. 0 ± 1. 4) mmol/L vs( 4. 8 ± 1. 3) mmol/L,( 3. 1 ±1. 0) mmol/L vs( 1. 2 ± 0. 6) mmol/L,( 2. 7 ± 1. 8) mmol/L vs( 1. 1 ± 0. 3) mmol/L,( 0. 65 ± 0. 26) mmol/L vs( 0. 43 ± 0. 12) mmol/L,( 4. 7 ± 2. 4) vs( 1. 2 ± 0. 4),( 9. 2 ± 1. 1) % vs( 4. 2 ± 0. 5) % ]( P〈0. 05 or P〈0. 01).Correlation analysis found that serum FFA was positively correlated with BMI,abdominal circumference,TC,TG,HOMA-IR,and Hb A1 c( r = 0. 330,0. 469,0. 215,0. 227,0. 248,0. 588,all P〈0. 05). Conclusion Serum FFA level is positively correlated with insulin resistance in patients with newly diagnosed T2 DM.
