摘要
门静脉血栓是肝硬化患者的一个常见并发症。随着影像技术的发展和应用,越来越多的门静脉血栓被发现。肝硬化门静脉血栓形成与肝硬化患者凝血功能紊乱、血流速度减慢以及血管内皮损伤有关。门静脉血栓的诊断除通过临床表现外,主要是通过影像学检查来确诊以及判断严重程度。门静脉血栓可产生许多并发症,引起或加重门静脉高压而导致曲张静脉破裂出血或腹水形成。广泛的门静脉血栓防治的管理现仍未能形成共识,抗凝、取栓、溶栓及经颈静脉肝内门体分流术在肝硬化门静脉血栓的治疗中取得一定的治疗效果。本文回顾近年关于肝硬化合并门静脉血栓形成的有关文献,从流行病学、发病机制、诊断和防治方面加以阐述和分析。
Portal vein thrombosis is a common complication in patients with cirrhosis. With the development of imaging technology,more and more portal vein thrombosis was found. The formations of the portal vein thrombosis were related with coagulation disorders,blood flow velocity and vascular endothelial injury. The diagnosis of portal vein thrombosis depended on patient＇s clinical manifestations,especially determined by imaging examination. Portal vein thrombosis could produce many complications,causing or exacerbating portal hypertension,variceal bleeding and ascites. Extensive portal vein thrombosis could even cause intestinal ischemia and necrosis,threaten the lives and health of patients. The prevention and treatment of portal vein thrombosis in cirrhosis is still not a consensus. The means of anticoagulant,thrombolysis and transjugular intrahepatic portosystemic shunt in cirrhosis with the portal vein thrombosis have achieved some therapeutic effects. In this paper,reviewed the literature about portal vein thrombosis in cirrhosis recent years were reviewed,and the epidemiology,pathogenesis,diagnosis and prevention of portal vein thrombosis were described and analyzed.
