摘要
目的探讨儿童全身应用糖皮质激素对眼压的影响,引起临床医生的重视,避免对眼部造成不应有的损害。方法回顾性分析212例全身应用糖皮质激素患儿的眼压,分析不同疾病、不同剂量应用的同种激素对眼压的影响。结果所有患儿随访6个月以上,在212例患儿中出现激素性高眼压19例,激素性高眼压的患病率为8.96%,其中包括肾病综合征13例(15.48%)、过敏性紫癜4例(5.56%)、肺炎2例(3.57%),不同疾病之间高眼压患病率差异有统计学意义(P<0.05)。将患儿全身应用甲泼尼松龙琥珀酸钠治疗的初始剂量分别为≤20 mg、21~40 mg、41~60 mg、>60 mg 4个组,4个组出现激素性高眼压的患病率分别是2.78%、4.65%、10.72%、22.22%,各组之间差异有统计学意义(P<0.05)。结论儿童全身应用糖皮质激素可以引起激素性高眼压,激素性高眼压与不同疾病及应用激素的初始剂量相关,对于全身应用糖皮质激素的患儿应密切观察眼压变化情况。
Objective To explore the effect of systemic use of glucocorticoid on intraocular pressure of children,pay more attention to the effect,and avoid unnecessary damage on eyes in children.Methods The data of intraocular pressure of 212 children treated with systemic use of glucocorticoid was analyzed retrospectively.The effects of glucocorticoid used for different diseases and different doses of glucocorticoid on intraocular pressure were analyzed.Results All the children were followed up for more than six months.Among 212 children,19 children were found with glucocorticoid-induced intraocular hypertension,the incidence rate was 8.96%,13 children（ 15.48%） with nephrotic syndrome,4 children（ 5.56%） with anaphylactoid purpura,and 2 children（ 3.57%） with pneumonia were included.There was statistically significant difference in the incidence rate of intraocular hypertension among the children with different diseases（ P〈0.05）.The children were divided into ≤20 mg group,21-40 mg group,41-60 mg group,and 60 mg group according to initial dose of glucocorticoid,the incidence rates of glucocorticoid-induced intraocular hypertension were 2.78%,4.65%,10.72%,and 22.22%,respectively,there was statistically significant difference among the four groups（ P〈0.05）.Conclusion Systemic use of glucocorticoid in children can cause glucocorticoid-induced intraocular hypertension,which is correlated with different diseases and initial dose of glucocorticoid,more attention should be paid to the children to observe the change of intraocular pressure.
