摘要
目的:通过分析2013-2014年吴忠市3家医院儿科门诊部分处方,分析儿科门诊合理用药的情况和影响因素。方法:在吴忠市中医院、同心县中医院和同心县人民医院,随机抽取儿科门诊处方10293张,采用合理用药国际指标评价其合理用药水平,并通过多元线性回归确定影响儿科门诊合理用药的因素。结果:3家医院儿科门诊均存在用药品种数和处方费用偏高、抗菌药和注射药滥用现象,抗菌药物处方比例依次为61.0%,35.8%和41.6%,注射处方比例依次为59.0%,40.8%和29.6%。确定了患儿年龄与性别、处方中成药占比均会影响儿科门诊合理用药情况。结论:门诊儿科处方用药存在明显不合理现象,患儿抗菌药与注射用药使用率远高于国际标准范围,建议采取干预措施。
Objective To describe the situation of pediatric drug use in 3 hospitals of Wuzhong,Ningxia,China,and to explore the influencing factors of rational drug use in pediatric outpatient clinics. Methods A total of 10293 prescriptions were randomly selected from 3 hospitals pediatric outpatient clinics( The Traditional Chinese Medicine Hospital of Wuzhong,The Traditional Chinese Medicine Hospital of Tongxin County,and The Peoples Hospital of Tongxin County) from 2013 to2014. For the purpose of analyzing pediatric prescriptions,international rational drug use indicators were used. To determine the influencing factors affecting rational drug use in pediatric outpatient clinics,we established the Rational Drug Use Scoring System and measured with a multiple linear regression model. Results Thereexistedexcessive number of drugs,high cost of prescribed drugs and abuse of antibacterial drugs and injectable drugs inthe pediatric outpatient clinics of these 3 hospitals.The prescription ratios of antibacterial drugs in The Traditional Chinese Medicine Hospital of Wuzhong,The Traditional Chinese Medicine Hospital of Tongxin County,and The Peoples Hospital of Tongxin County were 61. 0%,35. 8%,41. 6%,respectively and the prescription ratios of injectable drugs were 59. 0%,40. 8%,and 29. 6%. This study found that childrens age and gender,and the prescription proportion of traditional Chinese medicine would influence the rational drug use in the pediatric outpatient clinics. Conclusions There was an obvious irrational drug use in pediatric outpatient clinics in Wuzhong,Ningxia,China. The use of antibiotics and injectable drugs was much higher than the international standard,thus,interventions should be adopted.
