摘要
目的观察锁定钢板治疗肩关节脱位并肱骨近端骨折的术后疗效。方法选取2011年6月~2017年6月我院收治的90例肩关节炎脱位并肱骨近端骨折患者,按治疗方式分为研究组60例和对照组30例。研究组采用锁定钢板治疗,对照组采用三叶草接骨板治疗肩关节炎脱位并肱骨近端骨折。比较两组患者的术后优良率、Neer评分及术后并发症。结果治疗后,研究组Neer评分均有所提升,其中研究组的活动范围、疼痛评分高于对照组(P<0.05);两组解剖部位和功能评分差别不大(P>0.05),研究组Neer评分高于对照组(P<0.05);研究组优良状况较对照组好,且术后情况差者仅4例,术后优良率优于对照组(P<0.05)。研究组并发症发生例数较对照组少。结论采用锁定钢板治疗肩关节脱位并肱骨近端骨折治疗效果更好,术后并发症发生情况更少,Neer评分改善明显,治疗效果较理想。
Objective To observe the efficacy of locking plate in the treatment of shoulder dislocation and proximal humeral fractures.Method 90 shoulder dislocation patients complicated with proximal humeral fracture treated from June 2011 to June 2017 in our hospital were selected.The subjects were randomly divided into study group（60 cases）and control group（30 cases）.The study group adopted the locking plate.The control group adopted the trifolium-shaped plate.After surgery,Neer score,the incidence rate of complications and postoperative good rate were observed.ResultThe Neer score for study group were increased.The range of motion and pain scores for study group were significantly higher than that of control group（P〈0.05）.The anatomic locations and function scores for two groups were not significantly different（P〉0.05）.The Neer score for study group was significantly higher than that of control group（P〈0.05）.The incidence rate of complications for study group was lower than that of control group.After treatment,the study group just had 4 unsatisfactory cases.The good rate for study group was significantly higher than that of control group（P〈0.05）.Conclusion Compared with the trifolium-shaped plate,the locking plate has a better effect in shoulder dislocation patients complicated with proximal humeral fracture,can reduce the incidence rate of postoperative complications and improve Neer score.
