摘要
目的探讨K受体激动剂纳布啡联合丙泊酚在无痛胃肠镜检查的应用效果。方法选取2017年4至6月接受无痛胃肠镜检查的患者60例，采用随机数字表法分为纳布啡组（N组）和舒芬太尼组（S组），各30例，N组予纳布啡0．1mg／kg联合丙泊酚麻醉，S组给舒芬太尼01μg／kg联合丙泊酚麻醉，待麻醉成功后进行胃肠镜检查。记录胃肠镜检查时间、清醒时间、离院时间、两组丙泊酚诱导用量和总用量。记录检查期间发生心率下降、低血压、SpO2降低情况。记录清醒后肠蠕动次数、清醒后疼痛数字模拟评分（N1KS）和患者的满意度。记录患者不良反应包括检查期间呛咳和体动情况、在麻醉复苏室（PACU）发生头晕、腹胀和腹痛情况。结果与S组比较，N组肠蠕动次数明显增多（P〈005），术后NRS评分降低（P〈0．05），丙泊酚总用量减少（P〈0.05），离院时间缩短（P〈0．05），患者满意度提高（P〈0．05）。结论纳布啡联合丙泊酚是适合门诊无痛胃肠镜检查一种有效、安全的麻醉方法，值得在临床推广应用。
Objective To explore the clinical efficacy and safety of nalbuphine combined with propofol in painless gastrointestinal endoscopy examination. Methods Sixty patients aged 18-65 years who underwent painless gastrointestinal endoscopy examination from April to June in 2017 were randomly divided into nalbuphine （N） group and sufentanil （S） group with the digital table method as follows： The N group received 0.1mg/kg nalbuphine combined with propofol （ N group, n-30 ） , the S group was given sufentanil 0.1μ g/kg composite propofol anesthesia （ S group, n-30 ） . After anesthesia, painless gastrointestinal endoscopy examination was performed. Time of examination, anesthesia waking, recovery, hospital leaving, the induction dosage and total dosage of propofol were recorded in the two groups. The cases of heart rate decline, hypotension, and low SpO2 were recorded during the examination. The frequency of intestinal peristalsis was recorded, so were the Numeric rating scale （ NRS ） score and the patient＇ s satisfaction. The adverse reactions including body motion, cough during the examination, dizziness, abdominal distension and abdominal pain in PACU were recorded. Results The number of intestinal peristalsis in N group was more than that in group S （P〈0.05） , the postoperative NRS score was lower （ P〈0.05 ） , the total dosage ofpropofol was less （ P〈0.05 ） , the time of hospital stay was less than that of group S （ P〈0.05 ） , and the patients＇ satisfaction was higher （P〈0.05） . Conclusion Nalbuphine combined with propofol is a safe and effective anesthesia method, which is suitable for outpatient painless gastrointestinal endoscopy, worthy of clinical application.
