摘要
目的 :探讨自拟通脉方辅助rt-PA溶栓治疗溶栓时间窗外急性脑梗死临床疗效。方法:选取符合标准的溶栓时间窗外急性脑梗死患者100例,随机分为对照组和观察组,每组各50例,对照组给予rt-PA溶栓治疗,观察组在此基础上加用自拟通脉方治疗,连续用药14d。比较2组患者近期疗效,治疗前后NIHSS评分、GCS评分、病变中心区域相对脑血流量及脑血容量水平。结果:观察组患者近期疗效显著优于对照组(P<0.05);观察组患者治疗后NIHSS评分和GCS评分均显著优于对照组、治疗前(P<0.05);同时观察组患者治疗后病变中心区域相对脑血流量和脑血容量水平均显著高于对照组、治疗前(P<0.05)。结论:自拟通脉方辅助rt-PA溶栓治疗溶栓时间窗外急性脑梗死可有效减轻神经损伤症状,改善远期预后,并有助于恢复病变区域血流灌注水平。
Objective: To investigate the clinical effects of self-made Tongmai formula assisted with rt-PA throm- bolysis in the treatment of acute cerebral infarction for outside thrombolytic time window. Methods: 100 patients with acute progressive cerebral infarction were chosen and randomly divided into both group including control group(50 pa- tients) with rt-PA used alone and experiment group(50 patients) with self-made Tongmai formula on the basis of control group; and the clinical efficacy for short-term, NIHSS score, GCS score, central area relative cerebral blood flow and cerebral blood volume before and after treatment of both groups were compared. Results The clinical efficacy for short- term of experiment group were significantly higher than control group(P〈0.05). The NIHSS score and GCS score after treatment of experiment group were significantly better than control group and before treatment(P〈0.05). The levels of central area relative cerebral blood flow and cerebral blood Volume after treatment of experiment group were signifi- cantly better than control group and before treatment (P〈0.05). Conclusion Self-made Tongmai formula assisted with rt-PA thrombolysis in the treatment of acute cerebral infarction for outside thrombolytic time window can efficiently reduce nerve injury symptoms, improve long-term prognosis and be helpful to restore blood perfusion level in lesion area.
