摘要
目的探讨慢性阻塞性肺疾病急性发作(AECOPD)患者深静脉血栓形成发生危险因素。方法选取2015年8月至2017年11月我院住院收治AECOPD患者352例临床资料行回顾性分析,根据患者是否发生深静脉血栓形成分为血栓组和非血栓组,收集两组患者一般临床资料及可疑危险因素,采用统计学软件进行单因素和多因素Logistic回归分析,分析影响AECOPD患者合并深静脉血栓形成危险因素。结果单因素分析得出,年龄、体重指数、吸烟史、病程、心力衰竭、恶性肿瘤、静脉曲张、心房纤颤、住院时间在血栓组和非血栓组患者中比较有统计学意义(P<0.05)。多因素分析得出,年龄、吸烟史、病程、心力衰竭、恶性肿瘤、静脉曲张、心房纤颤、住院时间为影响AECOPD患者深静脉血栓形成发生危险因素。结论影响AECOPD合并深静脉血栓形成危险因素较多,临床中应根据相关危险因素针对性预防和干预,以降低AECOPD患者深静脉血栓发生风险。
Objective To explore the risk factors of deep venous thrombosis in patients with acute exacerbation of chronic obstructive pulmonary disease（ AECOPD）. Methods 352 case of AECOPD patients in our hospital during Aug.,2015 ~ Nov.,2017 were selected and divided into thrombosis group and non-thrombosis group,the general clinical data and suspicious risk factors of two groups of patients were collected. Statistical software was used to analyze single factor and multivariate Logistic regression analysis,so as to analyze the risk factors of AECOPD complicated with deep venous thrombosis. Results Univariate analysis showed that age,body mass index,smoking history,course of disease,heart failure,malignant tumor,varicose veins,atrial fibrillation and hospital stay in thrombosis group and non thrombosis group were statistically significant（ P 〈 0. 05）. The age,smoking history,course of disease,heart failure,cancer,varicose veins,atrial fibrillation and hospitalization time were the risk factors for deep venous thrombosis in patients with AECOPD. Conclusion There were many risk factors for AECOPD complicated with deep venous thrombosis. In clinical practice,prevention and intervention measures should be taken according to relevant risk factors,which could reduce the risk of deep venous thrombosis in AECOPD patients.
