摘要
目的利用化疗期间超声弹性成像指标构建乳腺癌新辅助化疗疗效的评估模型,探讨该模型适用价值。方法新辅助化疗后行手术治疗的102例乳腺癌患者,参考Miller&Payne分级方法分为有效组和无效组。分别于化疗前、化疗2个周期、化疗结束后进行超声弹性检查,建立疗效的预测模型并分析其价值。结果 102例患者中82例化疗有效。化疗2个周期时,有效组肿瘤最大直径、肿瘤内动脉收缩期血流峰值速度(PSV)、动脉血流阻力指数(RI)、病变侧与对侧横向剪切波速度比值(SWVr)均明显低于无效组(P<0.05),PSV、SWVr为患者预后的独立预测因素(P<0.05)。以化疗2个周期时PSV、SWVr构建的疗效评估模型,对患者疗效的预测ROC曲线下面积为0.913。结论化疗2个周期时PSV、SWVr能够有效预测乳腺癌新辅助化疗患者疗效。
Objective To construct a model to predict the clinical efficacy of neoadjuvant chemotherapyfor breast cancer based on ultrasound elastography,and to investigate the value of the model. Methods A total of102 patients with breast cancer treated with surgery after neoadjuvant chemotherapy were enrolled. According toMiller Payne classification,they were divided into effective group and ineffective group. Ultrasound elastographywas performed before the chemotherapy,2 cycles after the chemotherapy,and at the end of the chemotherapy,then the predictive model of the curative effect was established and verified. Results The chemotherapy was effec-tive in 82 cases out of the 102 patients. After 2 cycles of the chemotherapy,the maximum tumor diameter,systolicpeak velocity in tumor(PSV),arterial resistance index in RI(RI),and lesion side shear wave velocity to contra-lateral side shear wave velocity ratio(SWVr)of the effective group were significantly less than those of ineffectivegroup(P〈0.05). PSV and SWVr after 2 cycles of the chemotherapy were independent predictors of the prognosisin patients(P〈0.05). The area under the ROC curve for predicting the efficacy by the regression model made byPSV and SWVr after 2 cycles of the chemotherapy was 0.913. Conclusion PSV and SWVr after 2 cycles of thechemotherapy can predict the efficacy of neoadjuvant chemotherapy in patients with breast cancer.
