摘要
目的探讨声辐射力脉冲成像技术(ARFI)对甲状腺良恶性结节鉴别诊断的价值。方法分析120例患者138个结节的ARFI成像特征,将甲状腺结节声触诊组织成像(virtual touch tissue imaging,VTI)图像分为4类,评估VTI图像分布特征。应用声触诊组织定量成像(virtual touch tissue quantification,VTQ)技术检测甲状腺结节横向剪切波速度(SWV)并进行比较分析。结果 ROC曲线分析138个甲状腺结节VTI图像得出:黑色、蜂窝状图像联合诊断良恶性结节具有较高的敏感性(93.8%)、特异性(76.7%)、准确率(82.6%);恶性结节的Vm、Vrate与良性结节比较,差异均有统计学意义(P<0.05),绘制ROC曲线,曲线下的面积分别为:0.851、0.799;诊断临界点分别为2.75m/s、1.32。结论 ARFI技术诊断甲状腺恶性结节有较高的敏感性和特异性,在临床应用方面有良好的诊断价值。
Objective The aim of this study was to evaluate sensitivity and specificity of acoustic radiation force impulse ima-ging ( ARFI) in differentiation of thyroid nodules. Methods ARFI features of 138 thyroid nodules were analyzed. The VTI fea-tures were divided into 4 types. Virtual Touch tissue quantification ( VTQ) was used to quantitatively analyze the elasticity and hardness of nodules. Results VTI-black or cellular sample was used as the diagnostic criterium of malignancy. The sensitivity, specificity and accuracy rate in differentiation between benign and malignant thyroid nodules were 93. 8%, 76. 7% and 82. 6%respectively. The Vm and Vrate of the malignant nodules showed statistically significant differences compared with those of the be-nign nodules ( P 〈0. 05). According to ROC curve the best diagnostic critical point of Vm and Vrate was 2. 75 m/s and 1. 32 re-spectively;The areas under ROC curve of VTQ were 0. 851 and 0. 799 respectively. Conclusion Acoustic radiation force im-pulse imaging has high sensitivity and specificity in evaluating benign and malignant thyroid nodules and there is a high diagnostic value in clinical applications.
