摘要
目的研究某三甲医院手术分级管理持续改进的实施效果。方法提取某三甲医院2014年7月至2015年6月实施手术分级管理改进前的手术相关质量数据指标作为对照组,另选取2015年7月至2017年6月实施手术分级管理改进后的相同质量数据指标作为观察组。制定管理措施持续改进,对比改进前后手术相关质量指标的变化情况。结果观察组手术相关质量指标,包括手术量、术后10日内死亡率、非计划再次手术率、医疗纠纷发生率、I类切口手术抗菌药物预防使用率等比对照组均有明显改善。结论通过手术分级管理的持续改进,规范了手术医师的医疗行为,手术相关质量指标逐渐趋于科学合理,促进了医疗质量的提高。
Objective： To study the effect of continuous improvement in graded management of operation in a hospital of class A,grade one. Method： The related data were extracted from the hospital as the control group from July,2014 to June,2015 before the improvement,so were the same data in the observation group from July,2015 to June,2017. Continuous improvement of management measures were made with the comparison of the changes between the relevant quality indexes before and after the improvement. Result： Observation group quality indicators related to the operation,including operation amount,mortality within 10 days after operation,unplanned reoperation rate,incidence of medical disputes,type Ⅰ incision operation of the prevention rate of antibacterial drugs were improved significantly compared with the control group. Conclusion： The continuous improvement in the graded management of operation can standardize medcial behavior and improve medical quality,make the related quality indicators of surgery scientific and reasonable,promote the improvement of medical quality.
