摘要
目的探讨新生儿脐血血气应用于妊娠期高血压疾病孕妇对新生儿窒息评估的临床意义。方法将2014年1月至2016年12月在我院进行分娩的妊娠期高血压疾病孕妇中,选择300例作为研究对象,并将其按照高血压分级分成观察1组100例(妊娠期高血压)、观察2组100例(轻度子痫前期)及观察3组100例(重度子痫前期),同时将同期在我院进行分娩的100例正常孕妇作为对照组,分别采用新生儿脐血血气评估与新生儿1min Apgar评分对4组新生儿的窒息情况进行判定。结果不同组别新生儿的脐动脉血血气指标均存在明显差异,p H值、PO_2、BE及HCO_3^-水平由高到低顺序为对照组、观察1组、观察2组、观察3组,PCO_2水平由高到低顺序为观察3组、观察2组、观察1组、对照组;不同组别新生儿的1 min Apgar评分,评分<7与评分<4新生儿窒息发生率存在明显差异,1 min Apgar(阿普加)评分由高到低顺序为对照组、观察1组、观察2组、观察3组,评分<7与评分<4新生儿窒息发生率由高到低顺序为观察3组、观察2组、观察1组、对照组。不同组别窒息新生儿的脐血动脉血气指标存在明显差异,pH值、PO_2、BE、HCO_3^-由高到低顺序为对照组观察1组、观察2组、观察3组,PCO_2水平由高到低顺序为观察3组、观察2组、观察1组、对照组。结论新生儿脐血血气评估能较好反映妊娠期高血压疾病新生儿的窒息严重程度,且具有较高的敏感性,可作为预测妊娠期高血压疾病新生儿窒息的有效方式。
Objective To investigate the clinical significance of umbilical cord blood gas in neonatal asphyxia assessment in pregnant women with gestational hypertension. Methods 300 pregnant women with gestational hypertension who gave birth in our hospital from January 2014 to December 2016 were selected as study subjects and divided into observation group 1( 100 cases of pregnancy-induced hypertension),observation group 2( 100 cases of mild preeclampsia) and observation group 1( 100 cases of severe preeclampsia) of 3 groups. Meanwhile,100 normal pregnant women who delivered in our hospital during the same period were taken as control group,and neonatal umbilical blood gas and 1 min Apgar score were used for evaluating four groups of newborns asphyxia. Results There were significant differences in umbilical arterial blood gas index of different groups. The order of p H,PO2,BE and HCO3^- levels from high to low was control group,observation group 1,observation group 2 and observation group 3,PCO2 level from high to low order was observation group 3,observation group 2,observation group 1 and control group; newborns 1 min Apgar score 〈7 and 〈4 had significant difference in neonatal asphyxia incidence,1 min Apgar score in descending order was the control group,observation group 1,observation group 2 and observation group 3,score 〈7 and score 〈4 in ascending order in neonatal asphyxia was the observation group 3,observation 2 groups, observation group 1 and control group.There were significant differences in neonatal umbilical arterial blood gas index among different groups of asphyxia neonates. The order of pH value,PO2,BE,HCO3-- in descending order was control group,observation group 1,observation group 2 and observation group 3,PCO2 level from high Low order was the observation group 3,observation group 2,observation group 1 and control group.
