乳腺癌新辅助化疗前后MR扩散加权成像ADC值与Ki-67表达水平的相关性研究 被引量:15

摘要 目的探讨乳腺癌新辅助化疗前后MR扩散加权成像(DWI)表观扩散系数(ADC)值与Ki-67表达水平的相关性。方法回顾性分析54例行新辅助化疗的乳腺癌患者资料。患者于新辅助化疗前及化疗后均行DWI检查,并取得检查后病理组织测定Ki-67表达情况。对比新辅助化疗前后乳腺癌ADC值及Ki-67指数的差异,并分析新辅助化疗前后乳腺癌ADC值(ADC前、ADC后)、ADC变化值(△ADC)、ADC变化率(△ADC%)与Ki-67指数(Ki-67前、Ki-67后)、Ki-67变化值(△Ki-67)、Ki-67变化率(△Ki-67%)的相关性。结果新辅助化疗后乳腺癌ADC值较化疗前增高(P=0.000),Ki-67指数较化疗前降低(P=0.007)。ADC前、ADC后、△ADC%与Ki-67前、Ki-67后、△Ki-67%无明显相关性(P=0.862、0.292、0.562),△ADC与△Ki-67呈负相关(P=0.033,r=-0.291)。结论乳腺癌新辅助化疗前后ADC的变化值可以反映肿瘤细胞增殖活性的变化趋势,为新辅助化疗的疗效评估提供一定帮助。 Objective To study the correlation between ADC value and Ki-67 expression of breast cancer before and after neoadjuvant chemotherapy( NAC). Methods Fifty-four cases with breast cancer who were received NAC were included in this study. All patients were performed DWI scan before and after NAC,and then Ki-67 was detected in the pathological tissues. The ADC values and Ki-67 expression of breast cancer before and after NAC were compared. And relationship between ADCpre and Ki-67 pre,ADCpost and Ki-67 post,△ADC and △Ki-67,△ADC% and △Ki-67% were analyzed. Results The value of ADC in breast cancer increased after NAC( P = 0. 000),and expression of Ki-67 was decreased( P = 0. 007). No correlation was observed between ADCpre and Ki-67 pre( P = 0. 862),ADCpost and Ki-67 post( P = 0. 0292),△ADC% and △Ki-67%( P = 0. 562). However,only △ADC was significantly correlated with △Ki-67( P = 0. 033,r =-0. 291). Conclusion The change of ADC during NAC may reflect the change of proliferation of tumor cell,and that would be helpful for assessing the effect of NAC.
