摘要
目的探讨腮腺Mikulicz病CT及MRI特征。方法回顾性分析宁夏医科大学总医院经手术病理证实的8例腮腺Mikulicz病的临床及影像学资料,其中6例行CT平扫及增强扫描,2例行MRI检查,观察病灶的类型、形态、密度或信号、边界及强化表现。结果 6例患者行CT平扫+增强扫描,其中4例为多发结节或肿块型,病灶边界清楚,呈等、稍高密度,且不均匀,内可见囊变及钙化,增强扫描腮腺内结节或肿块实质部分明显强化;2例为弥漫浸润型,表现为双侧腮腺弥漫性肿大,密度不均匀性增高,内多发粟粒样结节,其中1例病灶见"砂砾"样钙化,病灶边界不清,增强扫描呈弥漫不均匀强化。2例患者行MRI增强扫描,均表现为腮腺内肿块,呈长T1、稍长T2信号,信号欠均匀,内见散在点状囊变信号,病灶边界清楚,增强扫描实质部分明显强化。结论腮腺Mikulicz病在影像学上有一定特征性,多发结节或肿块型需与腮腺常见肿瘤相鉴别,弥漫浸润型需与腮腺炎症鉴别。
Objective To analyze and study the CT and MRI features of the parotid Mikulicz disease. Methods We retrospectively analyzed the clinical and imaging data of 8 cases of Mikulicz disease of parotid gland confirmed by pathology in Ningxia Medical University General Hospital,including Six cases of CT scan and enhanced scan,2 cases of MRI examination. Imaging features including type,morphology,CT density/MR signal intensity,margin,enhancement feature were valuated. Results 6 cases with CT enhanced scanning,among which,4 cases were the multiple nodules or lump type. They were slightly higher density,the internal density was uneven,cystic areas and calcification were found,the boarder was clear. In post-enhancement scanning,the solid part of the density parotid lumps was enhanced. 2 cases of CT were manifested as diffuse infiltrative disorder,bilateral parotid gland were showed as diffuse enlargement,the density of parotid gland increased,showed multiple miliary nodules,calcifications were found in 1 case,the boarder was unclear,enhanced scanning was diffuse unevenly enhanced. 2 cases with MRI enhanced scanning showed multiple nodules or mass in parotid gland.The signal intensity was long T1,slight long T2,the signal was uneven,more cystic changes could be found,the boarder was clear. In enhancement scanning,lesions were significantly enhanced. Conclusion Mikulicz disease of the parotid gland in the imaging have a certain characteristic. Multiple nodules or lumps types need to be distinguished between common tumors of the parotid gland. Diffuse infiltration types need to distinguish with the diagnosis of parotid inflammation.
