摘要
目的通过测量气胸患者CT轴位图像某一特定层面的径线预估肺压缩百分比。方法回顾性分析80例做过多层螺旋CT扫描的气胸患者,排除胸廓畸形、胸腔积液、胸膜粘连、肺叶实变等干扰因素。通过容积再现(VR)和最大密度投影(MIP)图像重组得出胸腔体积和气胸量,从而得出实际肺压缩百分比Y。选择气管隆突层面为标准层面,测量该层面上垂直距离胸廓最大长度D,然后测量该条线上气胸上缘距脏层胸膜的垂直距离d,垂直方向距离比值X=d/D。通过SPSS软件分析肺压缩比Y关于垂直方向距离比值X的回归方程。选出拟合程度最大的方程进行验证。结果所有方程均具有统计学意义,左侧气胸最佳方程为:Y=0.156+1.831X-0.717X^2+1.479X^3,右侧气胸最佳方程为:Y=0.246+0.313X+2.615X^2。结论气管隆突层面的垂直距离比值对预估气胸患者肺压缩百分比有临床意义,其手段简单便捷,能帮助临床医生快速决策,适合推广于临床实践。
Objective To estimate the percentage of lung compression by measuring the specific dimensions of CT axial images in patients with pneumothorax. Methods The clinical data about 80 patients with pneumothorax who underwent multi-slice spiral CT scan were retrospectively analyzed. The interference factors including thoracic deformity,pleural effusion,pleural adhesions and lobular consolidation were excluded. The thoracic cavity volume and pneumothorax volume were obtained by volume rendering( VR) and maximum density projection( MIP) images to get actual percentage( Y) of lung compression. The distance D was maximum thorax vertically. Then the distance D from upper edge of the pneumothorax to the visceral pleura was measured according to the formula of vertical distance ratio X = d/D. The regression equation of the lung compression ratio Y with respect to the vertical distance ratio X was analyzed by means of SPSS software to select the equation with the highest fitting degree to verify. Results All the equations were statistically significant. The best equation for left pneumothorax was: Y = 0. 156 + 1. 831 X-0. 717 X2+ 1. 479 X3. The best equation for right pneumothorax was Y = 0. 246 +0. 313 X + 2. 615 X2. Conclusion The vertical distance ratio of tracheal cartilage layer has clinical significance for predicting the percentage of lung compression in patients with pneumothorax. The method is simple and convenient,which can help clinicians to make rapid decision,therefor,which is suitable for clinical practice.
