摘要
目的 CDIO代表构思(conceive)、设计(design)、实现(implement)和运作(operate),将其引入手术室新护士规范化培训过程,并探讨应用效果。方法选取2014年5月及2015年5月入科新护士分别作为对照组(29名)和观察组(27名),参照新护士规范化培训大纲及手术室新护士专科培训考核方案,观察组采用基于CDIO模式专科化培训,对照组采用传统模式培训,2组培训时间均为1年,采用两独立样本t检验比较2组考核成绩(理论和实践操作)、满意度(医生、同行、患者满意度)及职业认同感。结果观察组理论及操作成绩分别为(85.12±9.22)分和(95.21±7.61)分,对照组理论及操作成绩分别为(73.32±10.37)分和(84.46±9.85)分,2组比较差异具有统计学意义(t=4.506,4.588,P<0.001);2组医生、同行和患者满意度比较差异具有统计学意义(t=3.871,4.023,3.523,P<0.001);观察组护士职业认同感总分高于对照组(t=14.024,P<0.001),对照组职业认知评价、职业社会支持、职业社交技能、职业挫折应对和职业自我反思等5个维度得分均低于观察组(t=5.811,7.780,7.299,7.958,7.703,P<0.001)。结论 CDIO模式专科化带教能增强手术室新护士规范化培训效果,帮助新护士更快更好融入工作,提高医生、同行及患者满意度,增强新护士职业认同感。
Objective To introduce CDIO approach in the standardized training for new nurses in operating room and to explore its effect. Methods The experimental group consisting of 27 new nurses from May 2014 to May 2015 received the CDIO approach,while the control group consisting of 29 new nurses the traditional training methods in a 1-year standardized training. Nurse 's performance in theoretical and practice test, professional self-identity, patient, surgeon and peer satisfaction were assessed and analyzed by using the independent two-sample t-test. Results The scores of theoretical test 85.12±9.22 and practice 95.21±7.61 in the experimental group were higher than those in the control group 73.32 ±10.37 and 84.46±9.85 respectively(t=4.506, 4.588,P〈0.001); the difference of patient, surgeon and peer satisfaction in the 2 groups showed statistical significance( t=3.871,4.023,3.523,P〈0.001) and the total score of professional self-identity in the experimental group was higher than that in the control group( t=14.024,P〈0.001). The scores of occupational cognitive evaluation, occupational social support, occupational social skills, career frustration coping style and career self-reflection in the control group were all lower than those in the experimental group( t=5.811,7.780,7.299,7.958,7.703,P〈0.001). Conclusion The effect of standardized training for new nurses in operating room can be significantly enhanced by CDIO approach-based specialized teaching, which helps the new nurses to integrate into the operating room more quickly and better, improves doctor and patient satisfaction, and strengthens the professional identity of the new nurses.
