摘要 目的 探讨腺苷预处理对肺缺血再灌注 (I/R)损伤的保护作用。方法  72只健康大耳白兔随机分为 3组 ,每组 2 4只。Ⅰ组 :未行缺血再灌注处理 ;Ⅱ组 :行左肺缺血再灌注处理 ;Ⅲ组 :行左肺缺血再灌注处理 ,并于术前 10min经静脉给予腺苷。采用在体肺缺血再灌注损伤模型 ,于缺血 4 5min、再灌注 1h、2h、4h进行动脉血气分析、血浆及左肺组织丙二醛(MDA)、左肺组织过氧化物酶 (MPO)活性、左肺组织湿干重比 (W /D)的测定。结果 Ⅱ组再灌注 2h、4h动脉血氧分压显著降低 ,各时点血浆MDA、左肺组织W/D均显著升高 (与I组比较 ) ;Ⅲ组腺苷预处理后可明显改善上述指标变化。结论 腺苷预处理通过抑制多核中性粒细胞 (PMN)在肺内的聚集和活化 ,减轻肺血管内皮细胞损伤的程度 ,而防治肺缺血再灌注损伤。 Objective To study the effect of adenosine preconditioning against lung ischmia reperfusion injury. Methods The in vivo rabbit ischemia model with occlusion of left pulmonary hilus for 45 minutes was established. Seventy two white rabbits were divided into three groups: Group I( n=24 ) without ischemia reperfusion and Group Ⅱ( n =24) with ischemia reperfusion and Group Ⅲ( n=24 ) with adenosine intravenous injection at 10 minutes before ischemia reperfusion. The changes of left lung wet-dry weight, malondialdehyde and myeloperoxidase in the injury lung were measured in each group at the time of 1,2,and 4 hours reperfusion after 45 minutes of pulmonary ischemia. Results In group Ⅱ, partial pressure of oxygen in artery decreased evidently at 2 hours and 4 hours after reperfusion. Left lung wet-dry weight, contents of malondialdehyde and myeloperoxidase increased evidently after reperfusion ( P <0 05). Conculsion Adenosine preconditioning can prevent ischemia-reperfusion lung endothelium injury and improve postischemia-reperfusion lung function by inhabiting neutrophil sequestration and activation in lung.
