摘要
目的介绍本院在国际医疗卫生机构认证联合委员会(JCI)标准下,运用PDCA循环优化提高本院门急诊药房处方调配管理效果。方法回顾性分析本院2016~2017年门诊药房处方调配情况。组建PDCA小组,通过现状调查剖析现药房药品处方管理模式中存在的问题,采用PDCA循环管理办法结合JCI标准逐步进行优化,持续改进。比较采用PDCA循环改进措施前后本院门急诊药房的药品管理处方调配差错率的变化。结果采用PDCA循环改进措施后,处方调配差错发生率从2016年1月的0.335%下降到2017年12月的0.039%,比较差异具有统计学意义(P<0.05),低于既定目标值0.0050%。结论结合JCI标准分析门急诊药房的药品处方管理中存在的问题,有利于标准化的药品管理模式,运用PDCA循环法对降低处方调配错误事件发生率具有显著效果,可以保障院内用药安全,提高本院门急诊药房的药学服务水平。
Objective To introduce the management effect of application of PDCA cycle optimization for improving the prescription allocation of emergency pharmacy in our hospital under the Joint Commission of International(JCI) standard. Methods Prescription allocation of outpatient pharmacy in our hospital from 2016 to 2017 were retrospectively analyzed. The PDCA group was established to analyze the existing problems in the drug prescription management model of pharmacies through the current situation survey, and the PDCA cycle management method combined with JCI standard was adopted to gradually optimize and continuously improve. The error rate changes of drug management prescription dispensing in the outpatient and emergency pharmacy of our hospital before and after the adoption of PDCA cycle improvement measures were compared. Results After the adoption of PDCA cycle improvement measures, the incidence of prescription dispensing errors dropped from 0.335% in January 2016 to 0.039% in December 2017, the difference was statistically significant(P〈0.05), which was lower than 0.0050% of established target value. Conclusion Combined with the JCI standard, the problems in drug prescription management of outpatient and emergency pharmacy are analyzed, which is conducive to the standardized drug management model. The application of PDCA cycle method has a significant effect on reducing the incidence of errors in prescription dispensing, which can ensure the safety of hospital medication and improve the pharmaceutical service level of the outpatient and emergency pharmacy of our hospital.
