摘要
目的观察调强适形放疗联合榄香烯注射液治疗鼻咽癌的疗效,探讨其对患者血清转化生长因子β_1(TGF-β_1)、血管内皮细胞生长因子(VEGF)水平的影响。方法选取2011年4月至2014年4月在枣阳市第一人民医院肿瘤医学部放疗室治疗的鼻咽癌患者98例,按随机数表法分为观察组和对照组各49例。对照组采取调强适形放疗治疗,观察组采取调强适形放疗联合榄香烯注射液治疗,均持续治疗2个月。治疗2个月后比较两组患者的治疗效果,并比较两组患者的毒副反应发生率、治疗前与治疗2个月后的血清TGF-β_1、VEGF水平、治疗前与治疗后1年的生活质量评分(QLQ-C30)及治疗后1年、2年、3年生存率。结果治疗2个月后观察组患者的治疗总有效率为87.76%,明显高于对照组的65.31%,差异有统计学意义(P<0.05);治疗前两组患者的血清TGF-β_1、VEGF水平比较差异均无统计学意义(P>0.05),治疗2个月后,两组患者的血清TGF-β_1、VEGF水平较治疗前降低,且观察组低于对照组[(21.36±3.42)μg/L vs(28.40±3.75)μg/L、(262.53±11.48)ng/L vs(319.87±12.81)ng/L],差异均有统计学意义(P<0.05);治疗后1年,两组患者的QLQ-C30分值较治疗前升高,且观察组高于对照组[(57.24±5.28)分vs(50.79±4.86)分],差异有统计学意义(P<0.05);观察组患者治疗后的3年生存率为46.94%,明显高于对照组的24.49%,差异有统计学意义(P<0.05);观察组患者的毒副反应发生率为20.41%,明显低于对照组的40.82%,差异有统计学意义(P<0.05)。结论鼻咽癌患者予以调强适形放疗联合榄香烯注射液治疗可提高临床疗效,降低患者的血清TGF-β_1、VEGF水平,改善生活质量,延长生存周期,且安全性较好。
Objective To observe the efficacy of intensity modulated radiation therapy combined with elemene injection in treating patients with nasopharyngeal carcinoma and to investigate its effect on the changes of serum levels of transforming growth factor β1(TGF-β1) and vascular endothelial growth factor(VEGF). Methods A total of 98 patients of nasopharyngeal carcinoma, who admitted to Radiotherapy Room of Department of Cancer Medicine, the First People's Hospital of Zaoyang City from April 2011 to April 2014, were selected and divided into the observation group and the control group according to random number table, with 49 patients in each group. The control group was treated with intensity modulated radiation therapy, and the observation group was treated with intensity modulated radiation therapy combined with elemene injection, both for 2 months. After 2 months of treatment, the effect of treatment was compared between the two groups. The incidence of side effects and the level of serum TGF-β1 and VEGF before and after 2 months of treatment, the quality of life(QLQ-C30) score before and after 1 year of treatment and the survival rates after1, 2 and 3 years of treatment of the two groups were compared. Results After 2 months of treatment, the total effective rate of the observation group was 87.76%, which was significantly higher than 65.31% of the control group(P〈0.05).There were no significant differences in the level of serum TGF-β1 and VEGF before treatment between the two groups(P〈0.05). After 2 months of treatment, the levels of serum TGF-β1 and VEGF were lower in the two groups than before the treatment, and the observation group were respectively(21.36±3.42) μg/L and(262.53±11.48) ng/L, which were significantly lower than corresponding(28.40±3.75) μg/L and(319.87±12.81) ng/L of the control group(P〈0.05).
