摘要
目的：研究加减寿胎丸合四物汤对肾虚血瘀型复发性流产患者细胞免疫因子的影响。方法：选取从2015年8月—2016年12月本院收治的肾虚血瘀型复发性流产患者40例作为研究对象,以随机数字表法均分为两组。对照组服用黄体酮胶囊及地屈孕酮治疗;观察组在此基础上服用加减寿胎丸合四物汤治疗。观察两组患者治疗后的临床疗效,比较两组患者治疗前后中医临床证候评分,并检测两组患者D-二聚体、IL-10、TNF-α治疗前后水平变化。结果：观察组总有效率为98.00%;对照组总有效率75.00%,观察组临床疗效略大于对照组。两组经秩和检验,u=2.4951,P=0.0126。两组治疗后中医症状评分均有下降,观察组治疗后中医证候评分显著低于对照组（P〈0.05）。经治疗两组患者D-二聚体、TNF-α水平均显著下降,而IL-10水平则上升;观察组治疗后D-二聚体及TNF-α水平明显低于对照组,IL-10水平明显高于对照组（P〈0.05）。结论：加减寿胎丸合四物汤治疗肾虚血瘀型复发性流产患者安全有效,能有效改善肾虚血瘀型复发性流产患者的细胞免疫因子状态。
Objective： To study the effect of Modified Shoutai Pill and Siwu Decoction on cellular immune factors in patients with recurrent abortion due to deficiency of kidney and blood stasis. Methods： Forty cases of recurrent abortion due to deficiency of kidney and blood stasis in our hospital from August 2015 to December 2016 were selected and randomly divided into two groups.The control group was treated with progesterone capsule while the observation group was treated with Modified Shoutai Pill and Siwu Decoction. We observed the clinical efficacy of two groups after treatment,compared the clinical symptoms of TCM before and after treatment in the two groups,and detected the changes of the levels of D-dimers,IL-10 and TNF-α before and after treatment in the two groups. Results： The total effective rate of the observation group was 98% and the total effective rate of the control group was 75%,while that of the observation group was slightly higher than that of the control group. The two groups were analyzed by rank sum test,u = 2. 4951,P = 0. 0126. In two groups after treatment,TCM symptoms score decreased. The observation group＇s TCM syndrome score after treatment was significantly lower than that of the control group（ P〈0. 05）. After treatment,two groups D-dimers and TNF-α levels were significantly decreased,while the level of IL-10 increased. D-dimer and TNF-αlevels after treatment in observation group was significantly lower than those of the control group. The level of IL-10 was significantly higher than that of the control group（ P〈0. 05）. Conclusion： The treatment of recurrent abortion with kidney deficiency and blood stasis syndrome is effective and safe and it can effectively improve the status of cytokines in patients with recurrent abortion due to kidney deficiency and blood stasis.
