摘要
目的探讨宫腔镜下近端输卵管栓塞术治疗输卵管积水对患者体外受精-胚胎移植(IVF-ET)成功率的影响。方法选取因输卵管积水不孕行体外受精-胚胎移植的孕龄期妇女150例,随机分为对照组及观察组,各75例。对照组运用腹腔镜下近端输卵管根部结扎术进行IVF-ET前预处理,观察组运用宫腔镜下近端输卵管栓塞术进行IVF-ET前预处理。比较两组患者手术时间、并发症发生率、术前卵泡刺激素水平及IVF-ET妊娠结局相关指标。结果两组患者手术并发症、术前卵泡刺激素水平比较差异无统计学意义;观察组手术时间明显低于对照组(P<0.05)。观察组与对照组受精率及卵裂率比较差异无统计学意义;流产率及异位妊娠率对照组明显高于观察组(P<0.05);观察组临床妊娠率明显高于对照组(P<0.05)。结论宫腔镜下近端输卵管栓塞术具有创伤小、风险低、操作简单,提高临床妊娠率、降低流产率、异位妊娠率等优点,具有临床推广价值。
Objective To investigate the effect of hydrosalpinx on the success rate of in vitro fertilization and embryo transfer（IVF-ET） in patients with hysteroscopic tubal embolization. Methods 150 pregnant women were randomly divided into the control group and the observation group, with 75 cases in each group. The control group was treated with laparoscopic proximal ligation of the fallopian tube ligation before IVF-ET,and the observation group was treated with hysteroscopic proximal tubal embolization before IVF-ET. The operation time, incidence of complications, preoperative follicle stimulating hormone level and IVF-ET pregnancy outcome were compared between the two groups. Results There was no significant difference in operative complications and preoperative follicle stimulating hormone levels between the two groups. The operative time in the observation group was significantly lower than that in the control group（P〈0.05）. There was no significant difference in fertilization rate and cleavage rate between the observation group and the control group; the abortion rate and ectopic pregnancy rate in the control group were significantly higher than those in the observation group（P〈0.05）; the clinical pregnancy rate in the observation group was significantly higher than that in the control group（P〈0.05）. Conclusion Hysteroscopic proximal tubal embolization has the advantages of small trauma, low risk, simple operation, improving the clinical pregnancy rate, reducing abortion rate and ectopic pregnancy rate, and has the value of clinical promotion.
