摘要
目的探讨彩色多普勒超声(彩超)对不同类型肝癌诊断及术后随访的价值。方法术后病理诊断肝癌患者120例;其中,原发性肝癌75例,转移性肝癌45例。分析术前彩超检查对两种肝癌的诊断准确率。术后60例患者定期行彩超检查(观察组),60例未行彩超检查(对照组),比较两组术后1、3和5个月血癌胚抗原(CEA)和甲胎蛋白(AFP)水平以及5年生存率。结果彩超对原发性肝癌和转移性肝癌诊断的准确率相仿(95.8%vs.93.3%)(P>0.05)。原发性肝癌内部和周围血流信号检出率高于转移性肝癌(P<0.05)。术后3个月和5个月,对照组CEA和AFP水平高于观察组(P<0.05)。观察组5年无复发生存率和总生存率均高于对照组(P<0.05)。结论彩超诊断肝癌准确率高,对肝癌术后也具有良好的实时随访价值。
Objective To explore the diagnostic and postoperative follow-up value of colour Doppler ultrasonography in different types of liver cancer.Methods A total of 120 patients was pathologically diagnosed with liver cancer,of whom 75 cases were with primary liver cancer and 45 cases with metastatic liver cancer.The accuracy rate of preoperative colour Doppler ultrasonography in the diagnosis of two types liver cancer was analyzed.The postoperative follow-up with ultrasonography was performed in 60 cases（group A）and the other 60 cases were not followed up with ultrasonography（group B）.Serum levels of carcinoembryonic antigen（CEA）and alphafetoprotein（AFP）in 1,3 and 5 months after operation and 5-year survival rate were compared between two groups.Results The accuracy rate of preoperative colour Doppler ultrasonography in the diagnosis of two types of liver cancer was similar（95.8% vs.93.3%）（P〈0.05）.The detectable rate of internal and peripheral blood flow signals was higher in primary liver cancer than that in metastatic liver cancer（P〈0.05）.Serum levels of CEA and AFP were significantly higher in group B than those in group A in 3 and 5 months after operation（P〈0.05）.The 5-year relapse-free survival rate and overall survival rate in group A were significantly higher than those in group B（P〈0.05）.Conclusion Colour Doppler ultrasonography has higher accuracy rate in the diagnosis of liver cancer.The colour Doppler ultrasonography has a better value in real-time postoperative follow-up in the patients underwent surgery for liver cancer.
