摘要
研究益骨丸联合组合型人工肾对肾性骨病患者钙磷代谢及血清FGF-23的影响。方法 :45例患者分为益骨丸组(A组)、组合型人工肾组(B组)、益骨丸联合组合型人工肾组(C组)各15例,分别测定治疗前后血清钙(Ca2+)、磷(P3+)、甲状旁腺激素(i PTH)及成纤维细胞生长因子-23(FGF-23)水平,分析不同治疗方法对患者钙磷代谢以及FGF-23水平的影响。结果:(1)C组有效率显著高于A组,C组与B组总有效率比较差异无统计学意义,但总体分析发现,益骨丸与组合型人工肾在治疗肾性骨病方面可能具有协同作用。(2)C组血磷水平下降最为明显,其次为B组,但A组患者i PTH较治疗前显著上升,B组和C组患者无显著性变化,提示C组患者血磷水平的下降与血清i PTH无相关性,但进一步比较3组患者血清FGF-23水平后发现,A组血清FGF-23的水平较治疗前无显著性改变,B组和C组较治疗前显著升高,同时其治疗后血清FGF-3的水平C组较A组、B组亦显著性升高,具有统计学意义。结论:益骨丸联合组合型人工肾对终末期肾病患者具有较好的治疗效果,其作用机制可能与升高血清FGF-23水平,降低血P3+有关。
To study the effects of Yigu Pills and combined artificial kidney on calcium and phosphorus metabolism and serum fibroblast growth factor-23 (FGF-23) in patients with renal bone disease. Methods: Total 45 patients were divided into Yigu Pills group(group A),combined artificial kidney group(group B) and Yigu Pills and combined artificial kidney group(group C),15 patients in each group. The levels of serum Ca2+,P3+,IPTH and FGF-23 before and after treatment were respectively measured and the effects of different therapies on calcium and phosphorus metabolism and serum FGF-23 in patients were analyzed. Results: (1) The effective rate in group C was significantly higher than that in group A,there was no statis- tically significant between group C and group B. According to the overall analysis,there may be synergism between Yigu Pills and combined artificial kidney in the treatment of renal bone disease. (2)The level of serum phosphate in group C declined most obviously,followed by group B. The iPTH level after treatment in group A was significantly higher than before,while in group B and group C,there was no significant change. The results suggested that there was no relation between the decline of serum phosphate level and serum iPTH in group C. According to a further comparison of the serum FGF-23 level in the three groups, the level in group A had no statistically significant before and after treatment,while it was higher in group B and group C after treatment. The serum FGF-23 level after treatment in group C was significantly higher than that in group A and group B,which was statistically significant. Conclusion:The combination therapy of Yigu Pills and combined artificial kidney has a good therapeutic effect on end-stage renal disease pa-tients. Its mechanism may be related to the in- crease of serum FGF-23 level and the decline of serum phosphate.
