摘要
目的了解我院2011~2016年耐碳青霉烯类抗菌药物肠杆菌科细菌(CRE)分布特点和耐药性。方法采用纸片扩散法(K-B法)检测其耐药性,WHONET5.6软件统计数据。结果 2011~2016年共检出657株CRE菌,检出率6.8%(657/9667);CRE菌株的菌属呈多样化,其中阴沟肠杆菌(39.7%)、肺炎克雷伯菌(18.1%)、大肠埃希菌(17.8%)三种细菌的CRE检出率最高。CRE细菌分布在不同临床标本中,以痰液、呼吸道标本为主(43.5%,286/657),其次为脓液、伤口分泌物(21.8%,143/657),第三为尿液(14.0%,92/657),三者占总数的83.5%。CRE细菌分布在各个临床科室中且随着年龄的增大,CRE菌株的检出率也随之增加。CRE菌对各种抗菌药物的耐药率普遍较高,甚至对亚胺培南、美罗培南均出现了不同程度的耐药性,耐药率分别为18.4%、14.9%。结论 CRE菌常对多种临床常用的抗菌药物呈现耐药,且CRE的细菌种类不同,耐药表型及耐药性也不同。临床医生应该根据细菌耐药谱选择用药所以在临床治疗。
Objective To understand the distribution characteristics and drug resistance of carbapenem resistant Enterobacteriaceae(CRE) in our hospital during 2011-2016. Methods Disk diffusion method(K-B) was used to detect drug resistance,and using WHONET 5.6 software statistics. Results A total of 657 strains of CRE bacteria were detected in 2011-2016,and the detection rate was 6.8%(657/9667).The strains of CRE strains were diversified.Among them,the CRE detection rates of Enterobacter cloacae(39.7%),Klebsiella pneumoniae(18.1%) and Escherichia coli(17.8%) were the highest.CRE bacteria were distributed in different clinical specimens,with sputum and respiratory tract specimens(43.5%,286/657),followed by pus,wound secretion(21.8%,143/657),third for urine(14%,92/657),and three in 83.5% of the total.CRE bacteria were distributed in various clinical departments,and the detection rate of CRE strains increased with age.The resistance rate of CRE bacteria to various antibiotics was generally high,even to imipenem and meropenem,and the resistance rate was 18.4% and 14.9% respectively. Conclusion CRE bacteria are often resistant to a variety of commonly used antibiotics,and the bacteria of CRE are different,and their resistance phenotypes and drug resistance are also different.Clinicians should choose antibiotics according to the bacterial resistance spectrum.
