摘要
目的探讨沈阳市艾滋病自愿咨询检测（VCT）门诊求询的男男性行为者（MSM）的性社会网络特征与无保护肛交行为的关联性。方法在中国医科大学附属第一医院VCT门诊应用滚雪球方法招募MSM,进行面对面问卷调查并抽血检测艾滋病病毒（HIV）感染情况。结果共有402名MSM到VCT门诊参加艾滋病自愿咨询检测,其中266人（66.2%）参与了社会网络调查并提供信息。参与社会网络调查的MSM群体中高中及以下文化程度者占69.5%,未参与社会网络调查的MSM高中及以下文化程度者占52.2%,差异有统计学意义（χ^2=11.7,P=0.001）。MSM的平均性社会网络规模为4.5,平均性社会网络密度为0.5。266名MSM网络成员中,在过去3个月有150人（56.4%）与同性发生过无保护肛交。多因素Logistic回归分析显示,年龄≥30周岁[调整比值比（aOR）=1.9,P=0.025]、类中心网络规模=3.1（aOR=1.3,P=0.034）是沈阳MSM发生无保护肛交行为的独立影响因素。结论文化程度较低的MSM更愿意报告其性社会网络信息,年龄≥30周岁和类中心网络规模较小的MSM发生无保护肛交比例较高。
Objective To explore the association between unprotected anal intercourse（UAI）reported by men who have sex with men（MSM）and their sexual network characteristics at a voluntary counseling and testing（VCT）clinic in Shenyang.Methods A face-to-face interview and serological testing were conducted among MSM recruited by using snowball sampling in the VCT clinic of the first affiliated hospital of China medical university.Results During the study period,66.2%（266/402）MSM self-reported their sexual network information and undertook blood testing for HIV infection.The proportion of high school and below education level among MSM who selfreported sexual networks was significantly more than that among MSM who did not self-report sexual networks（69.5%vs.52.2%,χ^2=11.7,P=0.001）.The average scale of sexual networks was 4.5 and the average density was 0.5.A total of 150（56.4%,150/266）participants reported having had unprotected anal intercourse with men in the past 3 months.After confounding factors adjusted,age≥30 years（aOR=1.9,P=0.025）and classified network scale=3.1（aOR=1.3,P=0.034）were independent impact factors of UAI reported by the subjects.Conclusion Inadequately educated MSM are more willing to report their sexual network information.Age≥ 30 years and smaller sexual network scale of MSM are associated with higher proportion of UAI.
