摘要
目的:总结保留乳头乳晕的单孔腔镜皮下乳腺切除术治疗早期乳腺癌的美容效果、手术安全性及临床疗效。方法:纳入2017年2月至2017年9月治疗的12例早期乳腺癌患者,评估单孔腔镜皮下乳腺切除术术后3个月的美容效果评分,总结手术出血量、手术时间、引流量及手术并发症等。结果:手术切口2.5~5.0 cm,平均(3.8±0.8)cm。术后3个月美容效果评分9~12分,平均(11.0±1.0)分:其中非常满意5例(41.7%),比较满意7例(58.3%)。术后无乳头乳晕坏死、皮瓣坏死、血肿、感染、积液、假体植入失败等并发症发生。结论:保留乳头乳晕的单孔腔镜皮下乳腺切除术并发症发生率较低,手术切口短,且小切口隐藏在上臂能遮挡的部位,术后美容效果好;手术时间、出血量、引流时间均在可接受范围。
Objective： To offer the endoscopic single-port nipple sparing mastectomy（ NSM）,a new surgical technique,which completes NSM only through a single-port incision. The aim of this study is to evaluate the cosmetic outcomes,safety and efficacy of endoscopic single-port NSM for patients with early stage breast cancer. Methods： From Feb. 2017 to Sep. 2017,12 patients received endoscopic single-port NSM. All of them were diagnosed with stage Ⅰ or stage Ⅱ breast cancer. The study evaluated the cosmetic scores 3 months after the operation. The amount of operative bleeding,operation time,the volume of drainage and complications were also analyzed. Results： The length of incision ranged from 2. 5 cm to 5 cm,with a mean of（ 3. 8 ± 0. 8） cm. The cosmetic scores 3 months after the operation were 9 to 12,with an average of（ 11. 0 ± 1. 0）. 5 patients（ 41. 7%） were very satisfied and 7 patients（ 58. 3%） were somewhat satisfied for the postoperative appearance. No nipple areola necrosis,skin flap necrosis,hematoma,infection,hydrops and implant failure were found. Conclusions： Single-port endoscopic NSM is associated with few complications,short incision,which could be hidden by the affected arm,and very good cosmetic outcome. Meanwhile,the operative time,amount of operative bleeding and the time of drainage are acceptable.
