摘要
目的探究乳突根治联合鼓室成形术治疗胆脂瘤中耳炎的临床效果。方法选择84例2016年2月—2017年5月胆脂瘤中耳炎患者按随机表分组。对照组采用乳突根治手术治疗;观察组开展乳突根治联合鼓室成形术治疗。比较两组患者胆脂瘤中耳炎康复效果;术后平均住院时间、干耳平均时间;外耳道狭窄、神经性耳聋等的出现率。并比较治疗前后听力水平、生活质量SF-36水平。结果观察组患者胆脂瘤中耳炎康复效果比对照组高,P<0.05;观察组术后平均住院时间、干耳平均时间比对照组短,P<0.05;观察组外耳道狭窄、神经性耳聋等的出现率比对照组低,P<0.05。干预前听力水平、生活质量SF-36水平相似,P>0.05。干预后观察组听力水平、生活质量SF-36水平优于对照组,P<0.05。结论乳突根治联合鼓室成形术治疗胆脂瘤中耳炎临床效果确切,可有效加速术后康复,改善听力和生活质量,减少并发症的发生率。
Objective To explore the clinical effect of radical mastoidectomycombined with tympanoplasty in the treatment of cholesteatoma otitis media.Methods 84 patients with cholesteatoma of the middle ear from February2016 to May 2017 were randomly divided into groups. The control group wastreated with radical mastoidectomy, and the observation group was treatedwith radical mastoidectomy combined with tympanoplasty. The rehabilitationeffect of cholesteatoma otitis media in two groups was compared, the averagepostoperative hospital stay, the average time of dry ears, the incidence ofexternal auditory canal stenosis and nerve deafness were compared. Thehearing level and the quality of life （SF-36） were compared before and after thetreatment. Results The rehabilitation effect of cholesteatoma otitis media inobservation group was higher than that in control group, P 〈 0.05; The averagehospitalization time and dry ear average time in observation group were shorterthan those in control group, P 〈 0.05; The occurrence rate of external auditorycanal stenosis and nerve deafness in the observation group was lowerthan that in the control group, P 〈 0.05. The level of hearing and qualityof life before intervention was similar to that of SF-36, P 〉 0.05. Afterintervention, the SF-36 level of hearing and quality of life in the observationgroup was better than that in the control group, P 〈 0.05. ConclusionRadical mastoidectomy combined with tympanoplasty is effective in thetreatment of otitis media with cholesteatoma. It can effectively acceleratepostoperative rehabilitation, improve hearing and quality of life, reduce theincidence of complications.
