摘要
目的:系统性评价针灸联合推拿治疗枕大神经痛的有效性和安全性。方法:计算机检索Pub Med、CENTRAL、EMbase、CNKI、VIP、CBM和Wan Fang Data等数据库,搜集针灸联合推拿治疗枕大神经痛相关的随机对照试验,检索时限从建库至2017年10月10日。由2位研究者独立筛选文献、提取资料并评价纳入研究的偏倚风险后,采用Rev Man 5.3软件进行统计分析。结果:最终纳入4个研究,共309例患者,Meta分析结果显示:与单纯使用针灸或者正骨相比,正骨联合针灸治疗枕大神经痛有效率均更高[OR_(针灸)=4.70,95%CI(1.89,11.67),P=0.000 9;OR_(正骨)=11.46,95%CI(4.06,32.25),P<0.000 01]。结论:针灸联合推拿治疗枕大神经痛在有效性方面优于单用针灸或单纯正骨,治疗安全性较高,但由于纳入研究的证据质量级别较低,临床医生使用该证据进行临床决策时仍需谨慎。
Objective： Systemic evaluation bonesetting efficacy and safety of the combined acupuncture and moxibustion treatment of greater Occipital Neuralgia. Methods ： Computer retrieval PubMed, EMbase, CENTRAL, CNKI, VIP, CBM and Wan Fang Data database. Collect bonesetting combined acupuncture and moxibustion treatment of greater Occipital Neuralgia of randomized controlled trials, from building library retrieval time on October 10, 2017. By two independent researchers screening studies, extract information and evaluation into the risk of bias, using Rev Man 5.3 software for statistical analysis. Results： Four studies were included in the study, with 309 patients in total. The results of meta-analysis showed that, compared with the simple use of acupuncture or ortho- paicus, the combined acupuncture was more effective in the treatment of occipital neuralgia [ ORAcupuncture= 4.70, 95% CI （1.89, 11.67）, P = 0. 000 9 ; ORMassase = 11.46, 95% CI （4.06, 32. 25 ）, P 〈 0. 00001 ]. Conclusion： Acupuncture combined massage therapy pillow neuralgia in effectiveness is better than that of acupuncture alone or pure bonesetting, treatment of security is higher, but due to the low into the research evidence of the quality level, clinicians need to be careful while using the evidence for clinical decision.
