摘要
目的本研究采用文献研究探讨心脏神经症中医证候分布规律,以更利于阐释心脏神经症的中医证候分布特点,为临床辨证论治提供参考依据。方法采用横断面研究设计方案,首先通过清华大学CNKI医学知识数据库查阅21年文献资料,进行文献研究,运用描述分析方法统计各变量的频度和构成百分比,总结心脏神经症的中医辨证规律。结果文献研究显示:心脏神经症7个基本证候为:气虚、血虚、阴虚、血瘀、气滞、痰浊、阳虚,其中气虚证最为多见,其次为血虚与阴虚;心脏神经症的证侯要素组合主要有四类:单因素证,双因素证,三因素证及四因素证。其中以四因素证的气虚+阴虚+痰浊+血瘀和气虚+血虚+痰浊+血瘀最为多见;气虚在证候要素组合中分布最广。结论结合文献研究,气滞、气虚是心脏神经症的主要证素,气滞、气虚、痰浊、血瘀是心脏神经症的主要病理机制,心是心脏神经症的主要病位,肝、脾与之密切相关。
Objective This study used literature to study the distribution of TCM Syndromes of cardiac neurosis, in order to explain the characteristics of TCM syndrome distribution of heart neurosis and provide reference for clinical syndrome differentiation.Methods A cross-sectional study design scheme was adopted. Firstly, the literature of 21 years was consulted through the CNKI medical knowledge database of Tsinghua University, and the literature was studied. The frequency and percentage of the variables were calculated by the method of description and analysis. The TCM syndrome differentiation of the heart neurosis was summarized.Results Literature studies show that 7 basic syndromes of heart neurosis are qi deficiency, blood deficiency, yin deficiency, blood stasis, qi stagnation, phlegm, yang deficiency, among which qi deficiency syndrome is the most common, the second is blood deficiency and yin deficiency, and there are four types of syndromes of cardiac neurosis： single factor syndrome, double factor syndrome, three factor syndrome and four factor syndrome. Among them, qi deficiency and yin deficiency, phlegm turbidity, blood stasis and qi deficiency, blood stasis, phlegm turbidity, blood stasis were most common among four factors; qi deficiency was the most widely distributed in the combination of syndrome elements. Conclusion Combined with literature study, qi stagnation and qi deficiency are the main syndromes of heart neurosis. Qi stagnation, qi deficiency, phlegm turbidity and blood stasis are the main pathological mechanism of heart neurosis. The heart is the main disease of heart neurosis, and the liver and spleen are closely related.
