摘要
目的:评价临床药师抗感染会诊的效果,探讨可能影响会诊临床结局的重要因素。方法:基于贵州省人民医院临床药师抗感染会诊平台,采用横断面研究描述2011-2016年的会诊情况,并采用队列研究设计前瞻性随访会诊后患者的临床结局。采用多水平Logistic回归模型分析影响患者临床结局的相关因素。结果:2011-2016年某院临床药师抗感染会诊数量逐年递增。队列研究共纳入431例患者,会诊意见接受率为89.33%,治疗有效率为63.80%。多水平Logistic回归模型结果显示,患者年龄、会诊类型、临床药师会诊意见的采纳情况对患者临床结局的影响具有统计学意义(P<0.05)。临床医生采纳临床药师会诊意见能够明显提高感染治疗的效果[OR(比值比)=8.94,95%CI(置信区间):3.99~20.00]。结论:某院临床药师抗感染会诊已初见成效,但仍处于起步阶段,提高临床医师对会诊意见的接受度将是提高抗感会诊效果的关键。
OBJECTIVE To evaluate the effectiveness of clinical pharmacist consultation for infectious diseases and explore the important influencing factors.METHODS The study conducted in Guizhou Provincial People＇s Hospital included two parts：cross-sectional study and prospective cohort study.Multilevel logistic regression model was established to analyze the factors influencing the outcome of patients.RESULTS The cross-sectional study showed that the number of consultations increased from 2011 to 2016.The cohort study including 431 patients indicated that the acceptance rate of pharmacist suggestions and effect rate were 89.33% and 63.80%,respectively.The age of patients,the type of consultation,and the acceptance of pharmacist suggestions played essential roles in the outcome of patients.Adopting the suggestions of pharmacist would improve the prognosis[OR（odds ratio）=8.94,95%CI（confidential interval）：3.99-20.00].CONCLUSION The consultation service has been well accepted by clinicians and taken effect on the treatment of infectious diseases.Nevertheless,it is still in the early stage,and increasing the acceptance of pharmacist suggestions will be the key for improving patient outcome.
