摘要
目的利用免疫磁珠阴性富集法建立一种肺癌淋巴结标本中肿瘤细胞的分选和富集方法,探讨该方法对提高淋巴结微转移诊断率的可行性。方法选取原发灶为非小细胞肺癌且淋巴结病理阴性的患者,取新鲜淋巴结组织,制备淋巴结悬液,采用阴性法富集肿瘤细胞,联合应用免疫荧光和免疫荧光原位杂交技术鉴定肿瘤细胞,其中免疫荧光技术判断p53+/DAPI+/CD45-细胞为阳性细胞,免疫荧光原位杂交技术判断CEP8+/DAPI+/CIM5-细胞为阳性细胞。结果本研究入组30例肺癌患者,共收集病理阴性淋巴结60枚。7枚淋巴结免疫荧光法检测阳性,7枚淋巴结免疫荧光原位杂交技术检测阳性,其中有3枚淋巴结两种检测均阳性,18.3%(11/60)的淋巴结免疫磁珠阴性法检测阳性,来自10例患者,即33.3%(10/30)早期非小细胞肺癌患者存在淋巴结微转移。结论免疫磁珠阴性法可用于淋巴结微转移的研究,与常规病理检测相比,可提高肺癌淋巴结微转移诊断率。
Objective To establish an enrichment and detection method by immunomagnetic beads for disseminated tumor cells in lymph node specimens of lung cancer, and to explore the feasibility of this method to improve the diagnosis rate of lymph node micrometastasis. Methods Lymph node were ob- tained intraoperatively from lung cancer patients and turned into digest solution. All lymph node were patho- logically non- metastatic. Tumor ceils were enriched by negative method and identified by immunofluores- cence and immunofluorescence in situ hybridization. Only p53 +/DAPI +/CD45- cells and CEP8 +/DAPI +/ CD45- cells were judged as positive cells. Results Thirty patients with lung cancer were enrolled and 60 lymph nodes were obtained. Positive cells were detected in 7 lymph nodes by immunofluorescence and 7 lymph nodes by immunofluorescence in situ hybridization. Both p53 +/DAPI+/CIM5- cells and CEP8 +/DAPI+/CD45- cells were detected in 3 lymph nodes. Therefore, eleven lymph node from 10 NSCLC patients were found to be positive of micrometastasis. Conclusion Negative enrichment by im- munomagnetic beads is feasible to enrich disseminated tumor cells from lymph nodes and potentially leads to a more accurate estimation of lymph nodes micrometastasis for lung cancer patients.
