颅内脑膜瘤ADC值与Ki-67指数和肿瘤细胞计数的相关性 被引量:2

Correlation of ADC value with Ki-67index and cell count in intracranial meningioma

摘要 目的探讨颅内脑膜瘤表观扩散系数(ADC)值与Ki-67指数和肿瘤细胞计数的相关性。方法 78例病理确诊的脑膜瘤患者分成低级别组(WHOⅠ级,55例)及高级别组(WHOⅡ或Ⅲ级,23例)。术前行常规的MRI平扫、增强和弥散加权成像扫描,测量肿瘤直径最大层面的ADC值。术后所有标本测算Ki-67指数及肿瘤细胞计数。比较两组间ADC值、Ki-67指数及肿瘤细胞计数的差异,分析ADC值与Ki-67指数、肿瘤细胞计数的相关性。结果高级别组脑膜瘤ADC值低于低级别组(P<0.01);高级别组Ki-67指数及肿瘤细胞计数均高于低级别组(P<0.01)。ADC值与Ki-67指数(rs=-0.65,P<0.01)和肿瘤细胞计数均呈负相关(rs=-0.62,P<0.05)。结论 ADC值与Ki-67指数、肿瘤细胞计数相关,有助于术前鉴别高、低级别脑膜瘤。 Objective To investigate the correlation between apparent diffusion coefficient(ADC)value and Ki-67 index and tumor cell count of intracranial meningioma.Methods Seventy-eight cases with meningioma confirmed by pathology were divided into low-grade group(WHOⅠ,55 cases)and high-grade group(WHOⅡ orⅢ,23 cases).MRI routine plain scan,enhanced scan and diffusion weighted imaging scan were performed in all cases before operation.The ADC value was measured at the level with the largest diameter of meningioma.The Ki-67 proliferation index and tumor cell count of tumor specimens were detected.The ADC value,Ki-67 index and tumor cell count were compared between the two groups and the relationship between the ADC value and Ki-67 index and tumor cell count was analyzed.Results ADC value in high-grade group was significantly lower than that in lowgrade group(P〈0.01).The Ki-67 index and tumor cell count in high-grade group were significantly higher than those in low-grade group(P〈0.01).The ADC value was negatively correlated with Ki-67 index(rs=-0.65,P〈0.01)and tumor cell count(rs=-0.62,P〈0.05).Conclusion ADC value is associated with Ki-67 index and tumor cell count,which is helpful for preoperatively differential diagnosis of high-and low-grade meningiomas.
