摘要
目的探讨血清心肌酶指标乳酸脱氢酶(LDH)、天门冬氨酸氨基转移酶(AST)、α-羟丁酸脱氢酶(α-HBDH)、肌酸肌酶同功酶(CK-MB)和肌酸肌酶(CK)及脑脊液LDH、肿瘤坏死因子α(TNF-α)水平对神经内科颅内感染患者的诊断价值。方法选取医院2014年2月-2017年2月神经内科收治的58例颅内感染患者为研究组,按照性别相同、年龄相差不超过2岁的原则选择同时期在同一科室住院的116例未发生颅内感染患者为对照组;分析两组患者血清心肌酶指标LDH、AST、CK-MB、α-HBDH、CK水平及脑脊液中TNF-α和LDH指标水平。结果研究组血清心肌酶指标LDH、AST、CK-MB、CK和α-HBDH水平及脑脊液LDH和TNF-α水平均高于对照组(P<0.05);不同类型颅内感染患者血清LDH和CK水平及脑脊液LDH和TNF-α水平比较差异有统计学意义(P<0.05),血清AST、CK-MB和α-HBDH水平比较差异无统计学意义。结论血清心肌酶及脑脊液中LDH、TNF-α可作为颅内感染诊断的重要指标,同时血清LDH和CK水平及脑脊液中LDH、TNF-α水平检测有助于鉴别颅内感染类别。
OBJECTIVE To explore the value of serum myocardial enzyme indexes lactate dehydrogenase（LDH）,aspartate aminotransferase（AST）,α-hydroxybutyrate dehydrogenase（α-HBDH）,creatine kinase isoenzyme（CK-MB）,creatine kinase（CK）and cerebrospinal fluid LDH and tumor necrosis factorα（TNF-α）in diagnosis of intracranial infections in patients of neurology department.METHODS A total of 58 patients with intracranial infections who were treated in neurology department from Feb 2014 to Feb 2017 were chosen as the study group,and116 patients with the same gender and no more than 2 years of age who were hospitalized in the same department and did not have intracranial infection were set as the control group.The levels of serum myocardial enzyme indexes LDH,AST,CK-MB,α-HBDH and CK and cerebrospinal fluid TNF-αand LDH were observed and compared between the two groups of patients.RESULTS The levels of serum myocardial enzyme indexes LDH,AST,CK-MB,α-HBDH and CK and cerebrospinal fluid TNF-αand LDH of the study group were significantly higher than those of the control group（P〈0.05）.There were significant differences in the levels of serum LDH and CK and cerebrospinal fluid LDH and TNF-αamong the patients with the different types of intracranial infections（P〈0.05）,and there were no significant differences in the levels of serum AST,CK-MB andα-HBDH.CONCLUSION The LDH and TNF-αin the serum myocardial enzyme and cerebrospinal fluid can serve as significant indexes for diagnosis of the intracranial infections,and the levels of serum LDH and CK and cerebrospinal fluid LDH and TNF-αmay facilitate the differential diagnosis of the intracranial infections.
