摘要
目的探讨经鼻高流量湿化氧疗(HHFNC)在急性低氧性Ⅰ型呼吸衰竭患者中的应用效果。方法根据患者氧分压设置氧浓度分数,氧流量45~60L/min,温度34℃,维持患者血氧浓度至0.90以上,并给予相应气道管理,比较患者使用HHFNC前、后48h血氧指标及舒适度评分。结果 HHFNC治疗后患者呼吸频率、氧分压及舒适度指标显著优于干预前(均P<0.01)。结论 HHFNC可以有效缓解急性低氧性I型呼吸衰竭患者的低氧持续状态,配合有效的气道管理可提高患者舒适度。
Objective To explore the effect of humidified high flow oxygen therapy through nasal cannula（HHFNC）on patients with acute hypoxic respiratory failure in patients with acute hypoxic respiratory failure.Methods According to partial pressure of oxygen in patients with acute hypoxic respiratory failure,oxygen concentration fraction was adjusted,and high flow oxygen at a flow rate of 45-60 L/min,and at a temperature of 34℃ was administered to patients via a nasal cannula with an aim to maintain oxygen saturation over 0.90.Results of blood gas analysis and patients comfort scores were compared before and 48 hafter HHFNC.Results After the use of HHFNC,patients′respiratory rates,partial pressure of oxygen and comfort scores were significantly improved（P〈0.01 for all）.Conclusion HHFNC can effectively alleviate continuous hypoxemia in patients with type I respiratory failure of acute hypoxic nature.Airway management during the therapy can enhance patient comfort effectively.
