摘要
目的:探讨复方鳖甲软肝片联合恩替卡韦治疗乙型病毒性肝炎肝硬化的临床疗效。方法:选取2015年3月—2016年3月湖北省公安县人民医院接诊的乙型病毒性肝炎肝硬化患者68例,以不同治疗方案分为观察组和对照组,每组34例,对照组患者采用恩替卡韦治疗,观察组患者在对照组基础上联合应用复方鳖甲软肝片,比较两组患者的临床疗效、肝功能指标、血清纤维化指标及乙型肝炎病毒(hepatitis B virus,HBV)DNA水平。结果:治疗前,两组患者肝功能指标、血清纤维化指标及HBV DNA水平的差异均无统计学意义(P>0.05);治疗1年后,观察组患者丙氨酸氨基转移酶降低幅度明显大于对照组,白蛋白、凝血酶原活动度升高幅度明显大于对照组,透明质酸、层粘连蛋白、Ⅲ型前胶原及Ⅳ型胶原水平降低幅度明显大于对照组,平均HBV DNA水平明显低于对照组,上述差异均有统计学意义(P<0.05);观察组患者的总有效率为91.18%(31/34),明显高于对照组的76.47%(26/34),差异有统计学意义(P<0.05)。结论:复方鳖甲软肝片联合恩替卡韦治疗乙型病毒性肝炎肝硬化,可有效改善患者的肝功能和肝纤维化程度,且用药安全。
OBJECTIVE： To probe into the clinical efficacy of Fufang Biejiaruangan tablets combined with entecavir in treatment of liver cirrhosis of viral hepatitis B. METHODS ： 68 patients with liver cin＇hosis of viral hepatitis B admitted into People＇ s Hospital of Gongan County from Mar. 2015 to Mar. 2016 were selected and divided into observation group and control according to different therapeutic regimens, with 3,4 cases in each, the control group was treated with enteeavir, while the observation group was given Fufang Biejiaruangan tablets combined with entecavir. Clinical efficacy, hepatic function indices, hepatic fibrosis indices and the DNA level of hepatitis B virus （HBV）. RESULTS： Belbre treatment, there were no statistical significances in differences of hepatic function indices, hepatic fibrosis indices and the DNA level of HBV between two groups （P 〉 0. 05 ）; after 1-year treatment, the decreasing range of ALT of observation group was significantly larger than that of the control group, with significantly larger increasing ranges of AI,B and PTA; the decreasing ranges of HA, LN, PC m and C-IV levels were significantly larger than those of the control group, the average DNA level of HBV of observation group was significantly lower than that of the control group, all with statistically significant differenees（P 〈0. 05） ; the total effective rate of observation group was 91.18% （31/34）, which was significantly higher than that of the control group （76. 47% , 26/34）, with statistically significant difference （ P 〈 O. 05）. CONCLUSIONS： Fufang Biejiaruangan tablets combined with entecavir in treatment of liver cirrhosis of viral hepatitis B can effectively improve patients＇ hepatic function and hepatic fibrosis, with high medication safety.
